Whilst the Team leader was at the CP reporting the find, the three deminers – [Victim No.1], [Victim No.2] and [Victim No.3] - moved from their lanes to view the PMN. It was at this stage that [Victim No.1] stepped on the missed PMN. The missed mine was located approximately 1.5 metres from the marked PMN.

At approximately 08:58hrs an uncontrolled detonation of unknown item occurred on the ground between the feet of driver [the Victim]. Directly he and the medic [Name removed] rushed behind their team’s ambulance in unconscious response to the fear caused by the sound and blast of explosion.

GPM Support vehicle drove to the machine to handover the replacement fuze; the vehicle then reversed out back down the 'cleared lane' towards the 'safe area' and veered off this lane into an uncleared area (GPM skip area). The GPM Support vehicle had a mine-strike approximately 1m into the uncleared area. The vehicle was turned around 90º in a clockwise direction and resting approximately 2m away from the seat of the explosion.

On Saturday 02 October 2010 at approximately 10:00am a [Civil Contractor] grader struck an AT mine approximately one meter from the road center-line whilst carrying-out maintenance on the Juba to Nimule Road. The grader operator suffered serious injuries as a result of the blast. He was evacuated to a hospital in Juba, stabilized and then as he was from Uganda he was evacuated the following day to Kampala for further treatment. The grader operator later died from his injuries 20 October in Kampala.

According to the witness statements and the physical observation of the accident point, the accident happened while the de-miner was walking from the rest area to his clearance lane. He stepped on missed PMN anti-personnel mine in cleared area. Therefore, this mine was missed during the clearance operation. The exploded mine resulted in below knee amputation of deminer’s right leg, multiple injuries on his left leg, left foot heel, right hand palm and some minor injuries on his right arm. While the deminer was clearing an area assigned to him by the team leader and after he recovered an AT Saci mine he entered the contaminated hazardous area trying to locate the AP M14 12 o’clock mine with out following the SOP and the proper procedure , then he stepped on a non cleared area and activated the M14 which caused a badly injured to his left foot.

The accident occurred in (NR 205) on the 13th August 2010 in a cleared mine field (?) in Al Lafa Kassala where a tractor driver was injured by an Anti-Tank (AT) PRB 35 mine whilst cultivating an area of land that had been released through full manual clearance by [Demining group]. The tractor driver suffered injuries to his upper and lower right limbs.

On 13th August 2010 an Al Lafa, Kassala farmer was ploughing his field by tractor at approximately 2300hrs, when he detonated an antitank mine. The left rear tyre was destroyed, luckily he survived with surprisingly minor injuries. After some initial confusion as to whether the mine was or was not in a cleared area (perhaps a data reading error) and a preliminary investigation, it was established that the site had been previously cleared. BOI 2010/03 was initiated to determine the facts and make recommendations to the mineaction community.

It was 07:10 AM on 14 July 2010 while an anti-vehicle mine exploded under the back tyre of [Demining group] MDU-04 FEL machine during the ground processing operation. Fortunately no casualty is involved in this incident; only one tyre and a wheel were destroyed by the explosion. On the 22nd of June 2010 an uncontrolled detonation occurred at [Name of demining group removed] task site DA-SR-3948 in Central Equatoria within the confines of the control point. The detonation fatally injured the team medic and minor injuries were sustained to one of the team deminers’ who was in close proximity to the seat of explosion. The primary cause of this accident is listed as “Inadequate survey” because there seems to have been confusion about the area that required search. The secondary cause is listed as a “Management Control Inadequacy” because the failure to search the areas that were very obviously hazardous appears to have been the result of poor communication, a lack of adequate task documentation, a failure to keep MDD working records, poor understanding of Technical Survey, and other “shortcuts” that commercial demining groups may be expected to make when inadequately tasked.

The team leader [the Victim] was doing a QA check on one of the Foot Track already cleared by the deminer. Out of five mines laid only one M14 AP mine has been recovered from this FT. The team leader was trying to recheck the cleared area again with the metal detector trying to find the missing mine, during his QA check he stepped on one deep buried M14 AP mine in the area which has been already checked by the deminer and Team Leader.

On the 21st of April 2009 an UNMAO QA Officer from the Juba Sub-Office whilst conducting distance and bearing checks during a completion QA for the handover of a cleared DA-SS-796 in Central Equatoria State suffered serious injuries after stepping on an AP mine resulting in the traumatic amputation of his right foot. The injured was transferred to UNMIS medical facilities in Juba where he was stabilized and subsequently transferred abroad for further medical treatment. Task # 824 was one of the tasks surveyed by MCPA and then cleared by [Demining group] in period of around 10 months. The clearance operations started there on 12th December 2005 and completed on 17th November 2006. After completion of clearance operations and handing over of this task to local population, on 3rd of October 2008 a civilian mine accident happened to a 13 years old boy walking in the area busy in kite playing.

The BOI team believes this detonation may have occurred as a result of the deminer having leant forward from a safe point and thrown a marking stone down onto the ground. His aim may have been off and he may have inadvertently thrown the stone onto a mine that had not previously been found in the lane. The mine has detonated, propelling the marking stone and other stones back at him with the marking stone striking him in the chest and the remaining stones having caused the injuries around the forehead and eyes. Initial reports state that [the Victim] was working in his lane when a he initiated an unknown item. The size of the blast would indicate a partial or fuse assembly. A few seconds later, a larger explosion was heard, where [Name removed] had moved from his lane to assist [The victim]. It is the conclusion of the investigation team that the mine was not missed from the clearance team but had been brought by someone and put there. One of the local residents named [Name removed] narrated that, he had placed a mine under a small stone, but has not been found there. The crater made by exploded mine was less than 5cm which shows that as it was put on the ground [surface].

The deminer was working within the Site Preparation Stage to identify the centre of the mine line and an IOE was already identified and recovered the expected mines about 15m to our side from the mine centre line which already quality up to the assigned depth (15cm) and all the mines recovered from the site were a surface mines and when the deminer trying to bring out some stones he stepped on un expected mine in the site with the heel.

After the routine break at 1140 – 1150 hrs, the deminer was advised by his Section Leader (SL) to mark his lane clearly so that the SL could conduct his own QC checks. Just prior to the detonation the deminer was observed by the SL and another deminer to be preparing paint for the painting of rocks (for the next week). 22 October, 1045 hrs, as the tractor moves back down the track after completing the first plough run an explosion occurs under the rear left wheel, this splits the tractor in two and kills the driver. Map and photographs attached. Immediate action drills were carried out by the SEOD officer [Name removed], (who was at one end of the plough run and 58m from the blast and the tech survey team who were at the other end of the plough run and 176m from the blast.

On the 25th Jul 07 at approximately 1545hrs [Demining Group] reported to the MACC that they had located an amount of bulk explosive, 5-8kgs, at their HQ in Kudmose, Tyre. The explosive was located after a driver discarded a cigarette into tree/bushes adjacent to the [Demining Group] HQ office entrance. See photos at Annex B. A fire was initiated by the discarded cigarette, once the fire was extinguished an amount of explosive was then identified and located. This totalled approx. 5-7kg in weight.

On Thursday 5th of July 2007 at approx.1130 hrs, land caretaker ([Name removed]) informed one member of [International demining NGO] EOD team (via phone) that his workers ([Name removed] and his father [Name removed]) found one sub-munitions (1xM46), while working inside the citrus orchard, at Task CBU-193B.
