One person, [the victim]; aged twenty-seven was injured in the accident whilst conducting an intrusive visual surface search within allocated and marked boundaries as part of a Battle Area Clearance operation. After the accident he was transported to the Jabel Amel Hospital in Tyre where he was admitted and treated before being discharged after twenty – eight hours.

The LAF Sampling Team, was tasked by LMACC on the 17 August 2007 to conduct a reconnaissance of CBU 855 , Task Dossier, Area 1-001 in order to confirm the CBU turning points and areas for site setup. On the 25th Jul 07 at approximately 1545hrs [Demining Group] reported to the MACC that they had located an amount of bulk explosive, 5-8kgs, at their HQ in Kudmose, Tyre. The explosive was located after a driver discarded a cigarette into tree/bushes adjacent to the [Demining Group] HQ office entrance. See photos at Annex B. A fire was initiated by the discarded cigarette, once the fire was extinguished an amount of explosive was then identified and located. This totalled approx. 5-7kg in weight.

On Thursday 5th of July 2007 at approx.1130 hrs, land caretaker ([Name removed]) informed one member of [International demining NGO] EOD team (via phone) that his workers ([Name removed] and his father [Name removed]) found one sub-munitions (1xM46), while working inside the citrus orchard, at Task CBU-193B. The M46 was located hanging on the branch knob of lemon tree approx. 1.6 m from the ground level, in an area which had been cleared by [International demining NGO] using sub-surface Battle Area Clearance (BAC) procedures (Schonstedt GA-72Cd Magnetic Locator). On the 5th July 2007 at 1228hrs, two [International commercial demining agency] BAC searchers were involved in an accident, which was caused by an uncontrolled detonation of an M77 sub-munitions. On 2nd Jul 2007, two sub-munitions (1x M77 & 1xM42) were reported as found by the [International commercial company] completions team during a final inspection of CBU 771, before handover to the MACC SL. These items were located by the local land owner (woman) while harvesting her wheat crop. They were located in an area immediately adjacent to her house (within the task site) which had been reported as cleared by [International commercial company] sub-surface. The items were located on the surface. The owner reported the find to [International commercial company] completions team during and internal completion QA.

On the morning of 21st June 07 at approximately 0616hrs [Demining group] BAC Teams commenced operations at CBU 071. At approximately 1104hrs, an uncontrolled explosion occurred in the plantation at UTM 728309/3676516. The searcher was working in the lane when he had a need for toilet. The searcher closed his lane using a base stick (picture 1). The searcher put down his detector on the ground (picture 2). On 18 June 2007 at 1140 hrs (local time), [the Victim] a (Commercial demining group) Deminer, inadvertently caused an item of explosive ordnance (EO), which is considered to be a No. 4A anti-personnel mine, to activate, while conducting the manual mine clearance full excavation procedure. As a result of the explosion [the Victim] was thrown backwards by the blast wave and suffered a traumatic amputation of the right hand, a fractured right humerous and critical head and eye injuries.

CBU 19 was completed on the 31stMarch 07, see annex A for the [Demining group] Completion Report. According to the Completion Report the task was a combination of surface and sub-surface clearance in accordance with the operational clearance plan. The CBU was located, and reported to the MACC CLO, on Saturday (14th April) morning by a local farmer.

On the morning of the 9th April 2007 the land owner had contracted a local farmer to commence cultivation of his land for follow-up planting, during the process of the cultivation the operator of the machine noticed on his return ploughing an M77 sub-munition, had been turned over and was now quite visible on the surface on the newly ploughed area. He immediately stopped work and report the incident to the land owner who in turn informed {Demining group] personnel in the area.

1030hrs: Captain [Name removed], the team leader, directed 2nd Sergeant [the Victim], a deminer, to move the UXO about 2 meters from its original place to a pit surrounded by two big rocks for demolition. The deminer was ordered to put a 75 gram TNT block and an electrical detonator at the tail part of the UXO.
