This qualitative study examined how New York State licensed clinicians approach the initial assessment and ongoing treatment of older adults (55+) identified to be struggling with alcohol or substance misuse issues. The research questions specifically asked: Do clinicians assess for alcohol and substance misuse in the older adult patients they serve? What are the mediating factors within this assessment and treatment process? This study was initiated in an exploratory fashion because of the limited amount of research available which investigates the relationship between clinicians' attitudes and approaches to this work with the rapidly growing older adult populations they serve. The Elder Economic Security Standard Index (Elder Index) is a new tool for use by policy makers, older adults, family caregivers, service providers, aging advocates, and the public at large. Developed by the Gerontology Institute at the University of Massachusetts Boston and Wider Opportunities for Women (WOW), the Elder Index is a measure of income that older adults require to maintain their independence in the community and meet their daily costs of living, including affordable and appropriate housing and health care. The purpose of this study was to determine the food behaviors of nutritionally high-risk seniors as a function of their racial background, gender, marital status, and education level. A total of 69 seniors were identified to be at high nutritional risk using the Nutrition Screening Initiative (NSI) checklist. A supplemental questionnaire (SQ) was created to examine the risk factors in relation to the participant’s demographic background. Key results indicated that Asians practiced healthy food behaviors and women were more likely to eat alone (p≤0.05). Married participants (90.9%) were most likely to consume 2 meals or more each day. Development of tools for identifying and tracking differences in older adult service utilization by race, ethnicity, income, and geographic area is vital in the face of current demographic and economic changes in rural areas, particularly in areas experiencing rapid demographic changes. In this conceptual article, we explain how to calculate a service use index that compares service utilization of a specific group of older adults to that of the entire older adult population. The Community Living Assistance and Supports (CLASS) program, created under the Patient Protection and Affordable Care Act, established a federally-administered, voluntary insurance program that allows for working adults to purchase insurance to cover the cost of long term support services. The CLASS program is the first step towards moving away from a welfare-based system, improving consumer choice, and creating a stable funding source for long term care needs. Enrollees in the CLASS program who meet benefit eligibility requirements and need care assistance will receive a cash benefit to pay for supportive services such as home health care, adult day services, …

Older adults have an increased risk for illness and disease, factors that can lead to functional impairment and intensify the need for reliance on supportive services. Planning for long-term care needs is a vital component of healthy aging and continued autonomy. Yet, many older adults fail to make plans in advance, perhaps due to differences in personal characteristics.
The moderating effects of trait hope on the relationship between health-related quality of life (HRQoL) and preparation for future care needs (PFCN) was studied in a sample of 65 older adult primary care patients (<65 years) in western New York. Participants completed a questionnaire on 5 dimensions of PFCN (awareness of risk, information gathering, decisions about care preferences, concrete planning, and active avoidance). In addition, data were collected on five HRQoL domains (physical function, physical role function, emotional role function, social function, and bodily pain) and trait hope. Moderated multiple regression was used to test the moderator hypothesis controlling for personal characteristics. Post-hoc probing was used to further examine significant interactions.

The Lift Up Your Voice! (LUYV) training, a component of Community Catalyst’s effort to support the Campaign for Better Care (CBC), is designed to mobilize grassroots advocacy structures of vulnerable older adults by directly engaging and empowering older adults and their caregivers. The goal of the evaluation is to assess the effectiveness LUYV in recruiting potential advocates, educating them about the health care reform, empowering them via advocacy skills training, and engaging them in state-based CBC activities.

Social cognition is a fundamental aspect of human experience that enables us to have relationships with and understanding of other people. Social relationships have been shown to mitigate cognitive decline in old age and benefit cognitive functioning, and the social interaction on which these relationships rely requires an extensive network of cognitive processes, and by extension neural systems, that have not, as of yet, been widely studied in older adults. Many researchers have explored the way younger people perceive weight ratios using a variety of methodologies; however, very few researchers have used a more direct ratio estimation procedure, in which participants estimate an actual ratio between two or more weights. Of the few researchers who have used a direct method, the participants who were recruited were invariably younger adults. OBJECTIVES: Evaluate the effect of preadmission functional status on severity of pneumonia, length of hospital stay (LOS), and all-cause 30-day and 1-year mortality of adults aged 60 and older and to understand the effect of pneumonia on short-term functional impairment. DESIGN: Prospective cohort study. SETTING: University hospital. PARTICIPANTS: One hundred twelve patients with radiograph-proven pneumonia (mean age 74.6) were enrolled. MEASUREMENTS: Functional status and comorbidities were assessed using the Functional Autonomy Measurement System (SMAF) and Charlson Comorbidity Index. OBJECTIVES: Mortality rates from pneumonia increase steadily with age. Recently, a disease severity model (the Pneumonia Prognosis Index (PPI)) has been developed to predict mortality from community-acquired pneumonia (CAP). PPI ranks severity of pneumonia from 1 to 5, with 5 being most severe. This retrospective study utilizes the PPI to address the prognosis of CAP in older adults. DESIGN: Retrospective review of medical charts. SETTING: Department of Veterans Affairs Medical Center. Background: We aimed to examine the role of age and premorbid intelligence (IQ) in suppressing the relationship between subjective memory complaints (SMCs) and raw score memory performance. Methods: We used a community sample of older adults aged 66"90 years (N = 121) to test whether the inclusion of age and a premorbid IQ measure in multiple regression analyses increased semipartial correlations of raw score memory performance in predicting SMCs. Rank contrast correlations were also carried out to observe how age and premorbid IQ are related to complaint"performance congruency. The purpose of this article is to describe strategies that were effective in recruitment and data collection among older adults in 3 quantitative studies while decreasing costs in terms of time and money. Factors effective in reducing use of investigators' time and expenses included limiting exclusion of data because of abnormal Mini-Cog scores by careful initial screening and avoiding repeated reminders or follow-up, collecting data in small groups, collapsing consent, dementia screening, and data collection into single sessions, as well as accommodating for sensory and literacy deficits. Most workers adjust well to retirement, but some experience depression due to poor adjustment. No evidence-based treatments were identified that targeted the developmental needs of this complex transition. In phase 1, a treatment manual (IPT-RM) was developed. The manual comprised an adaptation of Interpersonal Psychotherapy. In phase 2, a pilot study was conducted to determine the treatment’s feasibility. A series of non-experimental AB singlecase studies was conducted with nine retirees. All research participants’ depressive symptoms receded into the non-depressed range by the end of the study. The IPT-RM treatment manual was developed to a Stage 1 level, which incorporated peer-reviewed …

Alcohol misuse increases risk of suicidal behavior in older adults. TheDepressive Symptom Inventory-Suicidality Subscale (DSI-SS; Metalsky & Joiner,1997) and its relation to suicide attempt history was examined to see if it differedfor older adults as a function of their alcohol use. Structural equation modelingwas used in a sample (N= 1,061) of older adult outpatients to examine the scale’smeasurement invariance and population heterogeneity and its relation to suicideattempt history. Analyses supported the equivalence of the DSI-SS in risky andnonrisky drinkers. The DSI-SS significantly predicted past suicide attempts. Find-ings support the viability of the DSI-SS as suicide screening tool for older adults.
