On 06 Sep 2010, when Mr. [the Victim] was working in his clearance lane and excavated a detected signal, during the excavation drill his excavation tool touched the mine and caused it to go off. On 1st of June [Demining group] started clearance operation on mentioned task, they cleared 3350 sqm and found/destroyed 150 anti-personnel and 90 UXO till the accident time. It is the conclusion of investigation team that the involved deminer was excavation a signal in an area where the ground surface was hard with dense vegetation, and considering the crater created as a result of the explosion it seems that the deminer was excavating the signal about 30 cm on right side of the working lane where he was not in a stable position for excavating the signal by scraper; the excavation of this lane required to be done in next clearance lane.

On 9th Oct 2007 at 08:10 Hrs, the accident happened on [Victim No.1] the Asst. Team Leader of [Demining group] EOD-4 while he was shifting UXO to pit. There were three antipersonnel PMN-2 mines near to the UXO and the Asst. Team Leader has stepped on one of these mines which in result the mine exploded and cut off his right leg and he also received some injuries on other parts of his body.

On the day of the accident at approximately 11:45hrs, the Victim was commencing a clearance lane up the side of a hill. He was investigating a reading from his metal detector by prodding, and was in the kneeling position at the time. Whilst prodding he detonated a PMA3 anti-personnel blast mine.

This accident occurred because the deminer activated a PROM mine, it is not possible to prove how the fuse was activated. In view of the fact that the deminers on the site were consistently exceeding established productivity rates by an excessively large amount, it is the view of the Board that the excessive operating speed was the major contributing factor.

On the day of the accident the victim was working in a bushy area when at 09:00 he pulled a bush out of the way. In doing so he pulled a tripwire and initiated a POMZ. He suffered hand and chest injuries, and hearing loss. The victim was treated at the site, then taken to the ICRC hospital in Ghazni for treatment.

The investigators determined that the deminer was "excavating" to investigate a detector reading when the accident occurred. He was using a shovel to do so. Investigators were unable to visit the accident site [due to deminer mobility and seasonal redeployment], so the report relied on interviews and written statements.

The investigators checked the Team's detectors and found five to be not "in proper working condition". The victim used one of these detectors. At the end of the working day the victim was taking his equipment to the store and either stepped into an uncleared area or stepped on a missed mine (due to the faulty detectors).

The Team Commander knew that there were mines present. "After thorough checking [he] had ordered the removal of 15cm of topsoil, followed by a second 15cm. The mine was still 15cm below the surface. This was too deep for the detector to pick it up and too deep for the prodder to reveal its presence… the ground compacted under his weight and set off the mine."

The investigators determined that the victim was squatting to prod with a bayonet and applied too much pressure. He used the presence of bamboo to explain why he squatted to prod. The mine was identified as a PMN from "found fragments". His visor was shattered in the accident and his bayonet was "lost".

An ex-pat deminer was checking the edge of “iron bomb of 16 inch Naval shell crater” in a minefield when he trod on a Type 72 anti-personnel mine. The mine has a minimum metal content and he suffered a traumatic amputation of his lower leg. It is presumed that he had his detector in his hand at the time of the accident.
