Empowering Providers To Empower Their Patients: One Model To Expand Knowledge, Competency, And Awareness For The Perinatal Substance Use Workforce, Jacqueline Jacobs

Perinatal substance use (PSU) is a serious and growing public health concern. It is associated with a variety of adverse health outcomes for both mother and child and has been shown to negatively impact the parent-child relationship. Despite the growing prevalence of PSU, there are notable deficits in provider knowledge regarding, and comfortability with, PSU. Moreover, providers report feelings of judgment, resentment, fear, and hesitancy related to their work with women with PSU. Subsequently, women with PSU struggle to find appropriate, compassionate, and effective treatment for their substance misuse. Widespread and accessible training is needed to bolster provider knowledge base, …

Previous research provides evidence that stigma can be perpetuated through language with consequences for well-being and quality of care. For example, providers who use stigmatizing language transmit bias toward patients with implications for care provided by other healthcare professionals. The current work extends upon this research by investigating perceptions of physicians who use stigmatizing or humanizing language. The current work sought to document the negative consequences of providers’ indelicate language on impressions of the provider, thereby motivating thoughtful language choices. Research shows a correlation exists between Adverse Childhood Experiences (ACEs) and poor mental health such as psychiatric and substance use disorders (Adshead, 2018). Research further suggests a correlation between ACEs and insecure attachment in adults (Adshead, 2018). The National Institute on Drug Abuse (2020) estimates 85% of the correctional population either has a substance use disorder or was incarcerated for a drug-related offense. This paper theorizes that those involved in the correctional system with a history of high ACE scores and substance use also experience insecure attachment, which is not currently addressed in correctional SUD programs. Substance use treatment addresses addiction behaviors, increasing coping skills, and sobriety but current modalities do not address the insecure attachment styles that may contribute to substance use for many individuals. Insecure attachment styles seemingly have a bidirectional relationship with substance use and are therefore are likely an important aspect of treatment. This case example focuses on an attachment therapist’s work with a young man with a substance use disorder and attachment insecurity. Attachment theory interventions are proposed to address potential contributors of continued substance use. The growing presence of electronic technology in the health service professions is redefining the boundaries of counseling services. Commonly referred to as telehealth, utilization of electronic communication strategies to expand connectedness has opened new frontiers in behavioral health through applications ranging from digital phones, interactive video sessions, to virtual supervision. Substantial research suggests that telehealth is generally equal in effectiveness to traditional forms of treatment, especially for those individuals struggling with substance abuse problems. The focus of this study was to examine the experiences of two past participants of an athlete-specific drug and alcohol rehabilitation program. Researchers have discussed the vulnerability of an athlete population and prevalence of substance use among athletes ( e.g., Turrisi, Mastroleo, Mallett, Larimer, & Kilmer, 2007; Martens, Watson, Royland, & Beck, 2005). The intention of this study was to better understand how certain aspects in a college student's life (i.e., perceived stress, styles of coping, and social support) or how combinations of these variables may contribute to higher levels of alcohol consumption. The present study examined the relationship between perceived stress, functional coping strategies, dysfunctional coping strategies, and perceived social support using Lazarus and Folkman's model of stress, appraisal, and coping. A sample of (N = 201) University of Denver undergraduate students between the ages of 18-25 complete measures of perceived stress, coping strategies, perceived social support, and alcohol use. Lawyers suffer from higher levels of anxiety and depression than the rest of the population, but most do not enter law school with these mental health issues. Disciplinary actions against attorneys involve substance abuse 50 to 75 percent of the time. However, neuroscience research has shown that both the brain and the genes enjoy the power of plasticity, which means that personal choices and environments shape the development of lawyers throughout their lives. Substance use regularly occurs among individuals that distribute drugs and there is a limited amount of research that has examined both use and distribution. A grounded theory approach was used to develop a framework for analyzing relationships between use and distribution behaviors. 15 participants (five female, 10 male) that had experience selling drugs and using drugs were recruited from a residential substance abuse treatment center. Each participant was interviewed for one hour with an interview protocol aligning with the stage of analysis. Alaska is known for its long, dark, cold winter months. Daylight savings time exists to cope with months that have little sunlight, but that still leaves on average, five hours of daylight. Special light bulbs exist to help with the darkness by providing light that tricks the body into thinking it is getting more sun than it really is. Family systems therapy is a way of working with individuals, couples, families, or a group of people that emphasizes relationships and a person's/family's greater "system" as important factors in establishing change and health (Haley, 1976; Minuchin, 1974).
