Background. Hepatitis C virus (HCV) infection engenders substantial metabolic changes. These changes are altered when the virus is cleared after successful treatment. We measured these metabolic alterations that occur after HCV cure; further, we assessed whether these alterations differed in subgroups defined by patients’ characteristics.

The overarching goal of this dissertation is to examine a recent rise in rates of deaths from suicide, alcohol, and drugs, labeled together as “deaths of despair” in the United States using Bourdieu’s theory of capital. Although the deaths of despair phenomenon is a product of a complex process related to various material and non-material factors, there is no theoretical framework that is able to account for different domains of predictors. In recent years, the increased availability of public data dashboards provided opportunities to democratize data access by summarizing large amounts of data into accessible snapshots to facilitate educational efforts and encourage evidence-based decision-making. However, many current practices have not reached their full potential because the dashboard design has not accounted for users’ needs and preferences. This dissertation employed the user-centered design framework with three studies. Pregnancy-related maternal morbidity and mortality disproportionately affect Black women more than their White counterparts. Black pregnant women are more likely to omit or engage in prenatal care late than White women. Social media is an essential source of pregnancy-related information and shows it effectively improves pregnancy knowledge. Greater than 80% of Black women own a mobile device, and some data demonstrate that Black women use social media for pregnancy-related information. Early in 2020, the COVID-19 pandemic became a global public health concern. College students became dependent on the online environment for learning, but also for receiving COVID-19 information. Understanding digital health literacy (DHL) in this unique population and subsequent prevention behaviors in a digitally connected population during a public health crisis is crucial to prepare for future pandemics. This study explored DHL in college students, their main sources of pandemic information and other information seeking behaviors, adherence to public health guidelines, and intentions to receive a COVID-19 vaccine. This study explores the role of medical discourse in the Iranian formal public sphere. It examines how an epidemiological wave of HIV/AIDS—known as "the third wave"—highlighted nontraditional sexual behaviors in public and enabled a shift in policy and discourse by the Islamic Republic State. Through analyzing published content on HIV/AIDS from five major Iranian newspapers between 2009 and 2013, this study identified four competing narratives of the third wave which coexist and have a dynamic relationship with one another. EXECUTIVE SUMMARYFor decades public administration and management scholars have conceptualized organizational networks as solutions to failures of markets and hierarchies (see, for example, Goldsmith and Eggers 2005; Osborne and Gaebler 1992). Relationships among organizations or actors within a network are framed positively as channels through which human, financial, and knowledge resources flow to address complex or “wicked” social problems (see, for example, Rittel and Weber, 1973). Background: Asian American family caregivers came from various ethnic groups with different sociodemographic characteristics and culturally specific values. However, cultural practices were often overlooked because researchers often combined all Asian Americans under one umbrella. Objective: The purpose of the study was two-fold. Latinx men who have sex with men (LMSM) are at significantly greater risk for HIV infection than their non-Latinx peers. One important way of helping prevent the spread of HIV is exposing vulnerable populations to HIV prevention programming. Little is known about what factors predict exposure to HIV prevention programming for LMSM. Given extant links between traditional masculine gender role norms and health outcomes, this study sought to examine possible relations between constructs of machismo, caballerismo, and heterosexual self-presentation, along with demographic covariates, and exposure to HIV prevention programming among LMSM. In this thesis, I take up the question of how the Medicare penalties introduced by the Hospital Readmissions Reduction program (HRRP) impacted the US health care system. The demand side of US health care system is highly fragmented where hospitals and other providers have to serve very different kinds of patients from different payers. Since the majority of a hospital's income is drawn from Medicare, a sharp reduction in its reimbursements is bound to have an effect on its health care delivery to all patients.

Obesity is a critical health and social concern. Almost 40% of American adults are obese, creating more than $300 billion dollars in annual medical costs. While obesity is a public concern that has been on the policy agenda for some time, obesity policies have proven difficult to formulate, adopt and implement in practice. In 2008, New York’s health commissioner asked LHDs and nonprofit hospitals to collaborate with community partners to conduct community health needs assessments, identify two or three health priorities to focus on, and implement evidence-based strategies to address them. The actions taken as part of the New York State Prevention Agenda mirrored the IRS rules and PHAB recommendations that followed. The dearth of published studies on public health-hospital collaborations and the momentum to understand how communities in New York have responded to the IRS rules and PHAB standards are the impetus for this research.

Across the U.S., concerns regarding substance abuse, addiction, and treatment are coming to the forefront of public discourse, due to increased media coverage of the popularly dubbed ‘opioid crisis.’ As overdose rates increase, particularly in rural and suburban white communities, so have debates about the ethics and efficacy of harm reduction services. Broadly construed, this thesis will examine the transformations in the socio-cultural construction of HIV/AIDS in Iran from the joint perspectives of media and medical sociology. Until recently, official public narratives in Iran primarily regarded AIDS to be a drug-related disease. However, beginning from the early 2010s, a new narrative frame (often called as “the third wave”) introduces a significant “alarming” collective shift in the transmissions modes of the disease toward sexual pathways. Those receiving medical care are at risk for developing serious hospital-acquired conditions (HACs), which include infections (HAIs) and other harm while receiving medical care. Pay for performance programs initiated by the Centers for Medicare and Medicaid Services (CMS), and private payers, linking reimbursements to quality of care, have created financial incentives for hospitals to address these HACs. The most common HAI in the United States, causing almost half a million infections, with 29,000 deaths within 30 days of diagnosis, results from Clostridium difficile. Infection with this bacterium leads to deadly diarrhea, and disproportionately impacts patients 65 and older, representing 80% …

Decentralization has become one of the most important public-sector reform programs globally since the 1990s with mixed results. In this four-paper dissertation, I explored how decentralization reforms affect public goods and services delivery, by analyzing how post-decentralization institutional changes shape local expenditure decisions and health service utilizations in Indonesia. Before the advent of anti-retroviral therapy (ART), many states enacted and strictly enforced laws that criminalized HIV transmission. These laws were enacted to prevent HIV-infected individuals from knowingly transmitting the virus to the uninfected. Over the past two decades, however, questions have been raised regarding the effectiveness of HIV criminalization and its unintended consequences. For example, there is little evidence that the criminalization of HIV transmission has been effective in reducing the spread of the disease, particularly when compared to education and other prevention efforts. It is well established that adverse childhood experiences (ACEs) increase risk of developing mental health disorders later in life. It is unclear how early in childhood these effects can be seen. Executive functioning (EF) and emotion regulation (ER) have been proposed as potential mechanisms which mediate the effect of ACEs on mental health. Individual ACEs have been associated with poor EF and ER. It is unclear whether ACE scores impact EF and ER in a graded, dose-response manner. Domestic violence has been shown to increase the risk of bullying behavior among youth. Substance use and excess weight have also been linked to bullying behaviors. The purpose of this study was to determine what anti-bullying interventions have been implemented throughout the United States, and to identify the role of three risk factors (domestic violence, excessive weight or substance use) on bullying behaviors. A decade of research has documented the adverse effect of multiple ecological risks on adolescent problematic Internet use (PIU), yet existing research provides rather segmental explanations, and few studies offer a comprehensive and integrated account. A better theoretical and empirical understanding of the associations of multiple ecological risks and PIU, as well as the underlying mechanisms, could help inform intervention practices and policies to mitigate adolescent PIU.

For persons living with diagnosed HIV infection (PLWDHI) 50 years of age and older, in addition to living with diagnosed HIV infection, they are also at risk for age-related chronic conditions. Optimal management of these conditions in addition to HIV is needed if they hope to achieve positive health outcomes such as achieving durable viral suppression, which is the goal for persons receiving HIV treatment. This dissertation comprises of five empirical essays. Essay one assesses the impact of prenatal care quality and its components on child mortality using nationwide data from the Zimbabwe Demographic and Health Surveys (ZDHS), 1999-11. Results indicate that increasing prenatal care quality by one unit improves neonatal, infant and under-five survival by about 42.33, 30.86, and 28.65%, respectively and all statically significant at the 1% level.

Using data from Behavioral Risk Factor Surveillance System (BRFSS), a telephone survey of adults in the United States, the first chapter of my dissertation studies the effect of state level and county level income inequality on health status and the pathway via public health spending. The risk factors for falls and the adverse outcomes of falls have been well documented among the general older adult population. However, fall frequency, fall risk factors, and fall outcomes have rarely been studied among minority groups specifically. Furthermore, to date there are no known studies that exclusively examine falls among Native American older adults.
