The issue I studied is the decision states have been afforded to expand Medicaid with the Affordable Care Act (ACA). I was curious to determine the health and economic impact on states that have and have not chosen to expand Medicaid. Specifically, I targeted Kentucky (expansion state) and Tennessee (non-expansion state). Tennessee and Kentucky are similar populations, have similar political inclinations, and are both very rural states. Introduction: The Affordable Care Act (ACA) increased access to health insurance throughout the United States. To date, an estimated 20 million previously uninsured individuals have gained access to coverage since the expansion. As a result, the number of fraudulent schemes reported has been on the rise. Among the many fraudulent activities in the healthcare sector, abuse of pharmacy benefits has been the most prevalent. The misuse and abuse of opioids, and opioid related overdoses has created a widespread epidemic throughout the country, thus extending opportunities for potential fraud within the pharmaceutical industry.

This paper provides a contextual review of research investigating associations between the health insurance reforms implemented by the Affordable Care Act and the accessibility and utilization of medication-assisted treatment for opioid use disorder. Research indicates that relative to 2008, the year of initial Medicaid expansions in 2014 is nationally associated with increased rates of insurance coverage, increased rates of treatment, and reduced rates of individuals with opioid use disorder forgoing treatment due to financial concerns (McKenna 2017). Many Americans face difficult financial decisions, often regarding the allocation of limited resources. Household resources may be split among the costs of food, clothing, housing, transportation, childcare, utilities, and health care. For some households, this allocation may present a challenge to purchasing sufficient food, leading to food insecurity. Most scholars agree the American South is a region with unique and distinct political traits. What is less clear, though, is just which states constitute “the South.” Are they the eleven former confederate states, as Key argues? Or are they the US Census Bureau’s version of the South?
