Social anxiety is a prevalent anxiety disorder marked by fear and discomfort in social situations where scrutiny or evaluation by others is anticipated. This thesis investigates the interplay between social anxiety and autism spectrum disorder (ASD), a neurodevelopmental condition characterized by social and communication challenges. Individuals with ASD often face heightened levels of anxiety, intensifying their social difficulties and impeding their social development. Early identification of social anxiety symptoms, particularly in ASD children, is crucial for timely intervention and improved outcomes. Parents play a substantial role in their children's emotional regulation (ER) abilities, promoting both adaptive and maladaptive development. Children with autism spectrum disorder (ASD) often have difficulties regulating their emotions, manifesting in externalizing behavioral issues (Mazefsky & White 2014). The present study examined the association between various child characteristics and maternal mental health concerns in three high-risk groups of mothers ± those of children with non-syndromic autism spectrum disorder (NSASD), mothers of children with fragile X syndrome (FXS), and mothers of children with fragile X syndrome and autism (FXS+ASD). Research has shown that mothers of children with NSASD and FXS are thought to share a degree of genetic vulnerability to mental health concerns it is key to examine the various other factors that may contribute to difficulties within the family system and maternal stress, depression, and anxiety.

This dissertation is comprised of two original research manuscripts broadly examining anxiety and potential correlates in young children with neurodevelopmental disabilities. The first manuscript assessed cardiac regulation during an auditory startle paradigm in young children (3-6 years old) with autism spectrum disorder (ASD), fragile x syndrome (FXS), and neurotypical peers. The first study is a within group analysis of the longitudinal development of ASD symptomatology in young children with FXS, and how diagnostic stability, language and non-verbal cognitive functioning may predict these trajectories. Pragmatic, or social, language skills play a pivotal role in abilities that facilitate interpersonal relationships and independence, particularly in adulthood. Pragmatic language deficits are considered a core trait of males with fragile X syndrome (FXS); however, mechanisms that influence these abilities are not well known. This research is also limited by a lack of valid assessments that adequately capture pragmatic language skills. The present dissertation is the first to examine pragmatic language skills and mechanisms that influence these abilities using a novel, dynamic language assessment, the Yale in vivo Pragmatic Protocol (YiPP) in young adult males with FXS. Objective: Autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), and anxiety are three of the most common childhood psychiatric disorders. Early trajectories of social avoidance have been linked with these psychiatric disorders in previous studies, but it remains unclear how social avoidance differentially predicts comorbid disorders in a high-risk genetic subgroup. Here, we delineate the association between trajectories of social avoidance from infancy and subsequent ASD, ADHD, and anxiety outcomes at preschool in children with fragile X syndrome (FXS), a well-characterized single-gene disorder highly associated with social avoidance as well as elevated rates of ASD, ADHD, and anxiety.

Background: Although aberrant visual attention has been identified in infants at high familial risk for autism, the developmental emergence of atypical attention remains unclear. Integrating biological measures of attention into prospective high-risk infant studies may inform more nuanced developmental trajectories, clarifying the onset and course of atypical attention and potentially advancing early screening or treatment protocols. Genetic liability to autism spectrum disorder (ASD) can be expressed in unaffected relatives through subclinical, genetically meaningful traits, or endophenotypes. This study aimed to identify developmental endophenotypes in parents of individuals with ASD by examining parents' childhood academic development over the school-age period. A cohort of 139 parents of individuals with ASD were studied, along with their children with ASD and 28 controls. Parents' childhood records in the domains of language, reading, and math were studied from grades K-12. Fragile X syndrome (FXS) and autism spectrum disorders (ASD) are characterized by deficits in social interactions (reduced eye contact, topic preoccupation, and social withdrawal) and maladaptive behaviors of anxiety, social withdrawal, and restricted interests (Kau, Tierney, Bukelis, Stump, Kates, Trescher et al., 2004). Prior research with individuals who have FXS has indicated that problem behaviors (i.e. social withdrawal and inattention) were correlated with abnormal activation of the hypothalamus-pituitary-adrenal axis (HPAA; Hessl, et. al., 2002); however, very few studies have investigated physiological patterns and associations with social dysfunction and restricted and repetitive behaviors for individuals with idiopathic ASD. The current study examined the association between specific child and maternal factors and parenting stress in three high-risk groups of mothers - mothers of boys diagnosed with idiopathic autism (IA), mothers of boys diagnosed with autism spectrum disorder (ASD) associated with fragile X syndrome (AFXS), and mothers of boys diagnosed with fragile X syndrome (FXS) alone. These three groups of mothers are thought to share some degree of genetic vulnerability to stress, as well as exposure to varying levels of challenging child behavioral characteristics.
