We examined the relationship of US cultural involvement with substance abuse and sexual risk behavior profiles from our nationally representative sample of public high school students in the Dominican Republic. Using a novel methodological approach to control for selection bias, we examined explanations for the so-called Latino or Hispanic immigrant paradox. A latent class regression analysis with manifest and latent covariates found that US cultural involvement indicators were independent and robust predictors of increased risk of co-ocurring substance abuse and sexual risk behaviors. Implications for prevention efforts targeting risk behaviors among Latino/a adolescents in the US and abroad are considered.

Research evidence shows that people who abuse alcohol are more likely to engage in behavior that puts them at risk of HIV infection, including having unprotected sex. In 2010, the Population Council’s APHIA II Operations Research Project collaborated with the Support for Addictions Prevention and Treatment in Africa Centre in Kenya on research to identify factors associated with alcohol and drug use among out-of-school youth in Kibera, to identify specific factors contributing to HIV risk, and to assess the impact of an intervention to link youths with an alcohol and drug abuse problem to counseling and rehabilitation services. This brief describes how screening for alcohol abuse and hosting long-term support groups through HIV testing and counseling centers (HTC) is one way to decrease high-risk sexual and alcohol behaviors among people who have tested for HIV. Findings from program activities funded by Population Council, Liverpool VCT and Support for Addiction Prevention and Treatment in Africa Center (SAPTA) found that screening HTC clients for alcohol abuse and referral to active support and counseling services is feasible. This scientific report on risk behaviors among adolescents in the Dominican Republic (DR) was prepared for the DR Ministry of Education. The study used a stratified cluster design of public high schools in the DR with sample weights. The questionnaire included items from the 2009 Youth Risk Behavior Survey. Results of this nationally representative survey were presented to a delegation from the DR Ministry of Education at Washington University's Brown School. Future orientation has been found to protect against risk behaviors in adolescents from diverse backgrounds. However, no studies have specifically examined future orientation as a potential protective factor against HIV risk behaviors in foster care adolescents. In this study, 343 foster care adolescents were interviewed about their future orientation, mental health, trauma histories, and cognitions related to HIV risk behaviors. Results indicated variability in future orientation, but there were no significant differences by race, gender, and age. Future orientation was significantly associated with mental health, trauma, HIV-related knowledge, attitudes, behavioral intentions, and number of sexual intercourse partners. In Kenya, an estimated 30 percent of men report to be alcohol users which may be fueling the HIV pandemic, due to its association with high-risk sexual behavior and reduced inhibitions. The Population Council’s Horizons program collaborated on a study to improve screening of clients for alcohol use, provision of feedback of screening results, and referrals to care and support during voluntary testing and counseling (VCT) for HIV. Overall, the results indicate that it is feasible to integrate alcohol risk reduction counseling into VCT, and that it is generally accepted by providers and clients alike. Given the high prevalence of HIV in Manipur, India and the critical role that female injecting drug users play in the transmission of HIV infection, it is becoming increasingly important to understand the sexual behaviors and drug use experiences of this group. This report describes the health status and patterns of treatment-seeking of female injecting drug users aged 15–34 in Manipur, and explores the factors underlying their vulnerability to HIV, including limited in-depth awareness of safe sexual behaviors and drug use practices, and limited self-perception of HIV risk.
