Vanderbilt University Medical Center (VUMC) launched a new electronic health record (EHR) in a “big bang” implementation that saw the new software go live across multiple hospitals, clinics and geographic locations in a single morning. The organization rightly focused most of its energy on preparing its nearly 25,000 employees for the impacts of the transition, but it also considered the effects that would be felt by its patients and families. Survey data indicate that patient satisfaction scores demonstrably dip before, during and after an EHR implementation, and take approximately a year to recover. A team at DMC employed a seven-step …

As the mother of a 28-year-old son with cerebral palsy and hydrocephalus, and as a healthcare consultant focused on patient experience and professional development, I have a unique perspective and skill set. Recently he experienced symptoms that included an excruciating headache, neck pain and lethargy. Fearing his ventriculoperitoneal shunt had malfunctioned, he went to the emergency room and was later admitted on the neuro inpatient floor for a three-day hospitalization. Social media sites are increasingly where individuals seek and share information on a range of topics. The focus of this thesis is on suicidal memes and how individuals interpret them. Suicidal memes are memes that suggest suicide through either text or visual images. No research has investigated interpretations of suicidal memes to date as well as why individuals post them. It is important to study such issues as the interpretations of these memes can mean the difference between depressed individuals, for example, receiving the help they need or those same individuals alienating themselves from their loved ones. This dissertation analyzes narratives (written and mediated) about widows’ post-loss experiences—specifically the ways in which these women embody and adjust/adhere to their post-loss widow identities—and whether or not the canonical/formula stories about widows reflect current experiences of widowhood. I look at older widowed women—both those in well-read widow memoirs and also in media portrayals of widows on film. The canonical view of widows as not attractive, not useful, and not interesting needs to be reexamined in light of changing ideas about gender roles and increased longevity. Surely older women have experiences, desires, and goals that encompass more than being socially …

Food allergies grew 18% from 1997-2007 (Centers for Disease Control and Prevention, 2013). As the prevalence of food allergies continues to grow, college students with diagnosed food allergies are having difficulty finding allergen friendly foods in college campus food service (Food Allergy Research & Education, 2014). In order to find the relationship between food allergy communication and prevalence of allergen friendly food on a college campus, a mixed method design was used for this research study. The purpose of this secondary data analysis was (1) to understand the use of a playbook as a positive distraction technique and (2) to explore the use of volunteers in the waiting room of an outpatient pediatric clinic setting. Specifically, the study examined the impact on perceived wait time, overall quality of care, and patient experience in a convenience sample of patients. Data obtained for a pilot program for improving patient experience were aggregated for exploratory analysis. This dissertation research explored the experience of partner dancing as a therapeutic process that reflects upon the dynamics and interactions of couples who are in the process of couple's therapy. The research also aimed to gain insight on the ways in which the experience of partner dancing can support these couples' therapeutic processes. Partner dancing is a dance-form that is based on the interaction between two individuals. Such interaction requires the dancing couples to communicate on many levels that may uncover layers of their relational dynamics. The Employment Update is sent via email to state agency contacts and a large stakeholder community, including individuals with disabilities, service providers, state associations, employers, advocacy groups, family members, representatives from academia and others. In 2009, the state of Oregon adopted its Employment First policy. When Oregon’s Office of Developmental Disabilities Services (DDS) decided to promote the implementation of this policy, it began by updating its existing employment website. The redesigned website (http:// www.dhs.state.or.us/dd/supp_emp/) emphasizes the value of integrated employment over other outcomes, and the importance of building community-wide conversations, with the goal of achieving integrated employment for people with developmental disabilities. The website shares the value of integrated employment with all targeted audiences by highlighting resources and illustrating employment successes. Electronic Nicotine Delivery Systems (ENDS) are non-combustible tobacco products that are rapidly gaining in popularity worldwide. ENDS are marketed as safer alternatives to cigarettes; however, very little research is available to support or deny these claims. ENDS aerosol is inhaled over the vocal folds and into the lungs consistent with cigarette smoke. The larynx is among the primary locations affected by smoking; therefore, it is necessary to evaluate the effect, if any, of ENDS on the larynx. The purpose of this research was to determine relationships among depression, anxiety, self-rated physical and mental health, self-advocacy, internalized homophobia, and quality of patient-provider communication to satisfaction with health care interactions. These were measured while controlling for select demographic variables: age; ethnicity; urban or rural domicile; relationship status; household income; highest educational attainment; health insurance; disclosure to health care provider as a gay man; reason for last healthcare visit; and, general health self-rating. People living with food-related illnesses find themselves subjugated by commonly held ideologies causing awkwardness in social situations. The current study is a qualitative analysis addressing how people with celiac disease (CD) navigate social situations in light of dominant beliefs that influence behaviors. Initially, I identify macro-level patriarchal, religious, sexist, ableist and etiquette-related commensality ideologies that disadvantage those with CD. The concept of positive deviance (PD) has not been studied in the context of nursing. Grounded in narrative inquiry and combined with vocabularies of motive and symbolic interactionism, the purpose of this study was to explore whether PD behaviors existed in nursing, and if so, to develop an operational definition of PD for nursing. The research question addressed what PD behaviors, if any, were present in nurses’ workplace stories. A traumatic brain injury (TBI) is trauma to the brain triggered by an external force that causes the brain to crash violently against the skull and disrupt normal functioning. Following a TBI, the survivor often faces challenges that compromise independence. These can include challenges in communication, cognition, and overall physical functioning. Resultantly, reliance upon the individual’s loved ones as both advocates and caregivers is essential to functional recovery. This qualitative study investigates the experiences reported between both survivors and their primary caregivers. Using a qualitative approach, exploration of the convergent and divergent perspectives between survivors and caregivers post-TBI were researched. …
