Using diet, exercise, consumer behavior, and demographic data from the National Health and Nutrition Examination Survey (NHANES), I examine the factors that contribute to body mass index (BMI) for the adult working age population. In Chapters 2 and 3, I examine how female BMI varies by race and ethnicity. In Chapter 4, I study the effect of food deserts on BMI and obesity for both males and females.

In Chapter 2, I use Bayesian Model Averaging (BMA) to examine the factors that influence female BMI for the adult working age population. Results indicate 12 of the 40 covariates are strongly …

This dissertation examines the welfare implications of vertical restraints in the U.S. health care market. I focus on two phenomena that influence vertical relationships between hospitals and insurers: vertical integration and vertical bundling. Both of these practices have potential efficiency-inducing and anti-competitive effects from a theoretical standpoint, making welfare implications ambiguous. I analyze their impact on welfare in an empirical setting. This task requires structurally modeling the market by using data on hospitals, insurers, and consumers, and simulating counterfactual worlds free of vertical restraints.

India has a backlog of nearly one million patients in need of cleft lip and palate repair. Unrepaired cleft results in social stigma and diminished health, psychological wellbeing, and academic functioning. Cleft repair surgery has the potential to restore functioning in these domains. However, the magnitude of the social, educational, and economic impact of cleft repair surgery has not been evaluated using statistically rigorous methods. However, the healthcare system in Nepal faces challenges such as limited access and lack of quality healthcare. Although there have been recent efforts to introduce universal healthcare coverage, there is limited evidence on existing systems to properly formulate a policy. With many individuals enrolled in Medicaid through the ACA Medicaid expansion and woodwork effect, this paper estimates the effects on utilization of hospital services and hospital finances. Using a difference-in-differences (DD) instrumental variables model, the results that suggest that increased Medicaid demand does not necessarily equate to increased profits.

Chronic disease prevalence among children and adolescents is rising, which is thought to result in part from elevations in allostatic load (AL). AL is the cumulative physiological dysregulation that results from exposure to biological, social and environmental stressors over time. Socioeconomic disparities in chronic disease and AL have been well-documented in adult populations, including links between childhood socioeconomic disadvantage (CSD) and AL, yet little is known as to whether CSD may begin to impact AL earlier in life. This dissertation is consist of three empirical studies in economics. The first part empirically examines the effect of universal pre-k on labor force participation of fertility age women in Oklahoma. I investigate the policy effect from both theoretical and empirical perspectives. I apply the synthetic control method (SCM) to the Current Population Survey (CPS) data to identify the causal relationship between universal pre-k and female labor outcomes. I find that universal pre-k increases labor supply of women aged 25 to 45 in Oklahoma. The second empirical study focuses on how the birth outcomes of US children respond to exposure to …

This dissertation consists of three empirical essays in development economics. In the first essay, I examine the impact of a health insurance scheme called the Rashtriya Swasthya Bima Yojana (RSBY), launched in 2008 in India, on schooling decisions and gender differences in education. At the outset, it is not entirely obvious as to whether health insurance would benefit education or have a detrimental impact. How do the underlying mechanisms of social norms and bargaining power relate to the acceptance of intimate partner violence within households? How do short run and long run determinants of gender norms affect attitudes toward IPV? This study begins to decompose the dynamics of the acceptance of IPV within couples using data from the Demographic Health Survey, as well as examine the relationship in the context of patriarchal societies using data from the Ethnographic Atlas. Cleft lip and/or cleft palate (CL/P) are birth conditions in which a newborn has an opening in the roof of their mouth or a slit in their upper lip. Left untreated, CL/P can lead to speech impediments and an unsightly orofacial deformity. However, a sequence of simple, common surgical procedures can restore function with minimal scarring. To date, the psychological effects of being born with CL/P have been investigated in the literature, but the restorative impact of CL/P surgery on psychological wellbeing has not been rigorously evaluated. Abstract: In the context of introducing biofortification of staple crops as a food-based approach to combat micronutrient malnutrition in Sub Saharan Africa (SSA), we carried out a survey in Tamale, Ghana to elicit consumers’ preference of and willingness to pay for one of the highly nutritious biofortified staple crop processed product: the Orange Fleshed Sweet Potato (OFSP) bread. Our results indicate that scores of respondents in the sample show preference for OFSP bread. The most important determinants of OFSP bread preference are its sweet taste and soft texture and consumers are willing to pay 0.634 pesewas more for the attribute …

Many health related issues in the United States are linked to Americans’ poor diet choices. College students, a subset of that population, establish important trends in their diet habits that they will maintain over their four years in college and beyond. Although previous literature has found income to be a determinant of diet, this paper does not find income to be a significant predictor of student nutrition. This dissertation consists of four papers in applied microeconomics / the economics of health and ageing that analyse the causal effect of public policies, or of events / issues amenable to policy intervention. Chapter 1 provides an overview of the papers in this dissertation.

Chapter 2 investigates whether a major and growing environmental disamenity – dengue fever – leads to protective behavior that increases residential electricity consumption. Being near a dengue cluster leads to a persistent increase in electricity consumption in 4-room and 5-room/bigger flats (by 1.7% and 1.1% respectively). Gun violence remains an important contributor to racial differences in mortality within the United States. Despite the existence of these significant racial disparities in firearm homicide victimization, the relationship between gun control policy and racial disparities in homicide remains largely unexplored within the empirical crime literature. In the two chapters contained within, my work investigates how human capital is formed and maintained through either individual decisions or historic events. As of January 2018, thirty-three states have adopted the Affordable Care Act (ACA) Medicaid expansion. There is an emerging literature to study the effects of ACA Medicaid expansion on various outcomes, such as insurance coverage, health status, and labor supply. For example, a study from the Congressional Box Office projects that the U.S. labor supply will decrease 1.5-2% from 2017 to 2024 because of this expansion. In a recent paper, Ayyagari (2017) argues that because employer sponsored retiree benefits are declining and Medicaid coverage is expanding, retirement age will decrease by 3.6 to 7.2 months. This dissertation comprises three essays on the economics of driving and health. The first examines the effect of the ride-hailing service Uber, its launch in New York City in May 2011, and the resulting drunk-driving outcomes in the counties that received service compared with those that did not. The difference-in-differences estimation of this effect implies a 25 to 35 percent decrease in the alcohol-related collision rate for the affected New York City boroughs, or about 40 collisions per month. With differentiated treatment effects for each affected county, the difference-in-differences effect is higher for Manhattan, middling for the Bronx and Brooklyn, …

The Effect Of Expansions In Medicaid Eligibility On Hospital Financial Performance: Evidence From Medicare Cost Reports, Kevin Wu

Expansions in Medicaid eligibility and effects on profitability for different hospitals are examined using data from Medicare Cost Reports and difference-in-differences analysis. Critical access and disproportionate share hospitals are associated with the largest profitability gains from these expansions. Results imply that vulnerable hospitals are affected most by changes to Medicaid.

This dissertation is a collection of three separate essays on the health of women and children. In the first essay, I along with my co-authors, analyzed the impact of two large, national-level health policies (the Janani Suraksha Yojana (JSY) and the National Rural Health Mission (NRHM)) on maternal health outcomes (proportion of institutional deliveries) in India. We used data from the India Human Development Survey (IHDS) and found that the JSY and the NRHM had a greater impact on institutional deliveries in high-focus states. Drug use disorders are considered chronic illnesses, like other severe mental disorders, and caregivers of drug users face a large impact on various aspects of their life. However, there is shortage of research in Myanmar about caregivers' experiences with caring for patients with drug use disorders. This mixed method study, therefore, aims at exploring the socioeconomic and psychological burden caregivers carry, the coping strategies they employ, as well as the barriers to coping they face. This paper conducts an analysis of county level data to determine the effect of fast food restaurants on type 2 diabetes rates. Due to endogeneity concerns with respect to the location of fast food restaurants, this paper follows the work of Dunn (2010) and uses the number of interstate exits in a given county to serve as an instrument for fast food restaurants. The strength of the instrument, which is theoretically and empirically tested in this paper, imposes some restraints on the interpretation of the findings. This dissertation consists of three chapters on the economics of social insurance. Each chapter explores an aspect of the evaluation and design of social insurance in terms of nutrition, healthcare and unemployment.

The first chapter, Kim, Fleisher and Sun (2016) report evidence of long-term adverse health impacts of fetal malnutrition exposure of middle-aged survivors of the 1959-1961 China Famine using data from the China Health and Retirement Longitudinal Study.
