This brief summarizes key results from the endline evaluation of the More than Brides Alliance (MTBA) project “Marriage: No Child’s Play” (MNCP) in India, Malawi, Mali, and Niger. The MTBA consists of partners Save the Children Netherlands, Simavi, Oxfam Novib, and the Population Council, along with 25 local implementing partners. The MNCP project—which took place from 2016 to 2020—aimed at being holistic and targeting pathways to child marriage on multiple levels simultaneously, treating communities as either having the full MNCP package or no intervention. In Malawi, Africa, the median age at first marriage is among the lowest on the continent and adolescent fertility rates are among the highest. Using high-frequency panel data from the country designed to follow single women and men into marriage, we examine the extent to which premarital fertility is associated with the timing of marriage. Two notable findings emerge. First, premarital fertility typically leads to a more rapid transition into marriage, compared to those not having had a premarital conception or birth, with controls. The effect is as strong for men as it is for women. Second, among women with …

Integration of depression screening and treatment into HIV care may prove critical to achievement of the 90–90–90 goals in Malawi and elsewhere in the region. Project SOAR has partnered with the Malawi Ministry of Health to implement and evaluate the integration of a depression management program into HIV care in Malawi. This brief focuses on initial results from the screening phase, which show that more than 90 percent of patients have been appropriately screened for depression. Important contributors to adolescent girls' and young women's (AGYWs') vulnerability to HIV are the power differentials in their sexual relationships with male partners. Therefore, engaging male partners of AGYWs is important in reducing HIV incidence among these women. The skills young people develop in school provide an essential foundation that can either be reinforced or deteriorate depending on the strength of the foundation and opportunities after leaving school. This brief describes findings from an analysis of a longitudinal data set of Malawian adolescents aged 14–17 attending school when first interviewed in 2007. The study investigated whether literacy and numeracy skills at school-leaving—among those who dropped out before completing secondary school—were retained several years after. Results show a significant gender difference in skill level after school-leaving for English skills, even after controlling for initial skill level and grade attainment, …

In response to a global policy effort to increase school enrollment, in 1994 Malawi became one of the first low-income countries to eliminate primary school fees. Since then, Malawi has achieved nearly universal primary enrollment, however enrolling young Malawians in school has not translated into keeping them in school. This policy brief describes the nature and consequences of school violence in rural Malawi—a common experience for both girls and boys. There is little evidence that school violence disrupts schooling as expected, with the exception of sexual violence experienced at school by boys. Violence at home is also common, and may …

In Malawi, and other resourced-constrained, sub-Saharan African countries with a high HIV burden, several service delivery models have emerged to increase prevention of mother-to-child transmission care retention by strengthening connections between health facilities and their surrounding communities. Yet there has been little documentation of the unique characteristics of each model and their comparative impact on mother-infant pair (MIP) care retention and other health outcomes. To better understand the epidemiological context for the DREAMS programming, Project SOAR analyzed previously collected quantitative cohort data from the Population Council’s Malawi Schooling and Adolescent Study (MSAS) to identify the prevalence and incidence of HIV and the herpes simplex virus, commonly referred to as genital herpes. SOAR’s analysis provides a deeper understanding of the changing epidemiological and gendered context of the HIV epidemic in Machinga. The youth cohort data from the MSAS provide accurate estimates of HIV prevalence over time and HIV incidence, thereby filling important data gaps.

Project SOAR is conducting implementation science research in Malawi to build needed evidence to inform decisionmaking about effective programming to reduce HIV risk among adolescent girls and young women (AGYW) and their male partners. This brief outlines a Project SOAR study which will provide critical evidence on the feasibility, effectiveness, and cost-effectiveness of an integrated depression treatment model for improving retention in and adherence to HIV care in Malawi. Among people initiating antiretroviral therapy (ART), those with depression are a large and especially vulnerable population. Depression affects 18–30 percent of patients receiving HIV care in Africa and is an important barrier to early ART retention. Evidence revealed by the study will have implications not only for the applicability of the model in Malawi but for other countries in sub-Saharan Africa as well.

In Sub-Saharan Africa (SSA), early and unintended pregnancy leads to a colossal loss of educational opportunities for girls. Existing studies that show associations between early/unintended pregnancy and school dropout lead to critical questions about how the education sector is responding to the issue in SSA. Conducted from August 2014 to April 2015, this review was devoted to an examination of such responses across six countries: Botswana, Kenya, Malawi, Tanzania, Uganda, and Zambia. There is global consensus that transmission of HIV from mother to child can be eliminated. Violence against women and children remains a major public health and human rights challenge in the world. This report provides information on a project evaluating the feasibility of having trained police officers safely and effectively provide emergency contraception (EC) to survivors of sexual assault as a means of broadening access to comprehensive care in Malawi. The effort capitalized on findings that the majority of survivors of sexual assault in Malawi report to the police first. Education, particularly at the primary level, has long been regarded as an essential input of social and economic development. In addition to its role in increasing the cognitive abilities of students, schooling is recognized as a facilitator of labor force participation, female empowerment, improved health outcomes, and greater civic engagement. As one of the early pioneers in sub-Saharan Africa of free primary education, Malawi’s enrollment figures compare favorably with those of its neighbors in southern Africa. Kenya and Malawi have developed national policies to prevent malaria in pregnancy. Measures to prevent malaria among antenatal clients were shown to be sustainable several years after pilot interventions ended. In Malawi, the approach has been expanded nationwide. In most malaria-endemic areas of Africa, women in their first and second pregnancies have the highest risk of acquiring malaria and, consequently, of malaria-associated anemia and low birth-weight. Two USAID-funded interventions aimed at strengthening the prevention and management of malaria in pregnancy were pilot-tested at the district level in Kenya (1998–2002) and Malawi (1998–2004). The Population Council initiated its work on adolescents in the mid-1990s. At that time, those advocating greater attention to adolescent issues were concerned about adolescent fertility—particularly outside of marriage—and adolescent “risk-taking” behavior. As an international scientific organization with its mandate centered around the needs of developing countries, the Council sought a more nuanced and context-specific understanding of the problems confronting adolescents in the developing world.
