In recognition of the need to reverse gender disparities, India has instituted numerous policies, laws, and programs intended to empower women and to protect women from violence. However, evidence on what works and what does not work to change notions of masculinity and femininity, reverse the widespread acceptability of marital violence at community level, and reduce women’s experience of intimate partner violence remains limited in the country. This report details results of a program implemented by the Population Council, together with the Centre for Catalyzing Change and the London School of Hygiene and Tropical Medicine: Do Kadam Barabari Ki Ore (Two Steps Towards Equality). The primary objective of the program was to test whether strengthening existing village-level self-help groups (SHGs); orienting members on violence against women and girls, and supporting them in prevention activities; and helping women who experience violence had changed their gender-role attitudes and reduced the experience of marital violence. On the whole, findings show that the program was acceptable and effective in many ways. …

Married young women and girls are the primary users of youth-centered sexual and reproductive health services in Ethiopia and, given the health risks associated with early and closely-spaced pregnancies, represent an especially important population to reach with sexual and reproductive health services. This brief looks specifically at the needs and preferences for family planning and reproductive health services among married young women and girls, with recommendations for how to more effectively ensure that they have access to those services.

This study examines the behavioral, sociocultural, economic, programmatic, and legal factors that aggravate Egyptian women’s vulnerabilities to HIV/AIDS. The study concludes that in low-prevalence settings like Egypt, targeted interventions are the most strategic and cost-effective: for example, HIV awareness-raising with a special focus on the preventive role of consistent, correct condom use, and creating a cadre of peers that could provide psychosocial support and HIV counseling to migrant workers. Furthermore, premarital and antenatal care programs should include HIV awareness-raising and provider-initiated counseling and testing after screening of women to identify those at risk for HIV infection. The gender profile of the HIV/AIDS epidemic has changed since it emerged 30 years ago when infections were concentrated in key populations of men such as injecting drug users and men who have sex with men. In 2012, women accounted for nearly 50 percent of the estimated 35.3 million people living with HIV/AIDS globally. In the Middle East and North Africa, 44 percent of infected adults are women. The past 30 years have also witnessed remarkable improvements in access to high-quality information and medical services, including services for prevention of mother-to-child transmission of the virus. Most mainstream HIV-prevention programs focus on increasing knowledge related to HIV transmission and risky sexual behaviors. These initiatives often do not take into account gender issues or power dynamics, nor do they address the risk that marital partners face within their relationships. Such programs assume that marriage is a safe haven for women and that their HIV risk is minimal. In response, the Population Council launched the “Kalkidan” project to enhance couple communication, promote HIV prevention within marriage, reduce stigma and violence, and increase demand for HIV information and services, including prevention of mother-to-child transmission—which will collectively contribute to HIV-negative …

This working paper reports on a study using Egyptian survey data to determine whether the effect of women's work on their status is mediated by the material transactions that accompany marriage. This perspective posits that marriage payments made to the bride, as well as assets she herself brings to the marriage, enhance her bargaining position. Analysis of the 2006 Egypt Labor Market Panel Survey panel data is used to explore the associations between women’s labor market activity, earnings, absolute and proportional marriage payments, and a measure of women’s status within marriage, namely their decisionmaking power. This working paper examines the influence of household poverty experienced during early childhood on early marriage and outcomes in schooling and workforce participation during adolescence for girls in Nepal. Much of the evidence concerning these relationships is drawn from cross-sectional data that cannot be used, and has not been able, to address causality. This Population Council study uses longitudinal data from the Nepal Living Standards Survey (NLSS), a two-wave panel in which the waves were conducted eight years apart to address these questions. Analyzing the data by household-wealth quintiles reveals surprisingly nonlinear results indicating that these associations are largest for …

Men have traditionally been excluded from initiatives related to reproductive health, family planning, parenthood, and sexuality. While HIV programs frequently recognize the role of men’s risk-taking behavior in transmission of HIV, little is known about men’s sexual and reproductive health. In Ethiopia, the Ethiopia Ministry of Youth and Sports, the Amhara Regional Bureau of Youth and Sports, and the Population Council have been implementing programs for vulnerable girls in Ethiopia, including married adolescents. This working paper analyzes parents’ decisions about girls’ schooling in the context of marriage through in-depth exploration of case studies in two rural areas of northern Bangladesh. The villages are sites of a long-term community study from 1991 and 2002, a time when significant changes were underway, partly as a result of new school incentive programs introduced in 1994. The data show that the rise of dowry demands, a relatively recent practice that is barely a generation old among Muslims in these areas, asserts an important and independent influence on marriage decisions and indirectly influences decisions about schooling. The Population Council undertook a large-scale study of married young women and men in two rural settings in India to assess their situation and vulnerability to HIV and other adverse sexual and reproductive health outcomes. Findings of the study clearly underscore the vulnerability of married youth to STI/HIV as a result of risky sexual experiences before, within, and after marriage. Furthermore, there is a need to sensitize healthcare providers to the special needs and vulnerability of married young people and orient them to the need for developing appropriate strategies to reach diverse groups of young people, including married young women …

This working paper explores the relationship between marriage arrangements and daily activities of young married women, using detailed time-use data from an adolescent study in rural Bangladesh. Measures of marriage arrangement are payment of dowry and the relative wealth status of natal and marital families. Using multivariate regression analysis, the results show that women’s time spent in domestic work, socializing, and self-care is significantly associated with marriage arrangement variables. Those who paid dowry spent more time in domestic work and less time in self-care and socializing relative to those who did not pay dowry. These patterns of association are similar …

In Pakistan, the contraceptive prevalence rate is 28 percent, unmet need for family planning services is 33 percent, and unwanted pregnancies are on the rise. A national study showed a fairly high induced abortion rate of 29 per 1,000 married women of reproductive age, and 1 out of 6 pregnancies resulted in induced abortion. This study explored how contraception and induced abortion are perceived as options for avoiding unwanted births by Pakistani men and women, to what extent they deliberately choose one over the other, and the language they use to talk about reproductive behavior decision-making. Berhane Hewan (meaning “Light for Eve” in Amharic) is a program in rural Amhara region designed to assist unmarried girls by imparting the knowledge, skills, and resources they need to avoid child marriage as well as give support to adolescent girls who are already married. With support from UNFPA, DFID, and the Turner Foundation, the program is implemented by the Ministry of Youth and Sport and the Regional Youth Bureaus, with technical assistance from the Population Council. This paper explores the potential importance of marriage and childbirth as determinants of school-leaving in sub-Saharan Africa and identifies some of the common underlying factors that contribute to premature school-leaving and early marriage and childbearing. Results suggests that the reproductive health community should see early marriage as a central area of concern for adolescent reproductive health. Policies that inform parents about the value of starting their children in school on time are likely to have beneficial effects both for grade attainment and for adolescent reproductive health regardless of school quality.

The timing of first union merits investigation not only because of the close temporal link between marriage and the onset of childbearing, but also because the age when men and women marry has implications for the organization of family life and for gender relations within society.
