End of life care for people with advanced chronic disease is a growing international imperative, with the majority of deaths in the world now related to chronic disease. The provision of care that meets the needs of people with advanced chronic disease must be guided by appropriate policy. The key policy areas impacting directly on end of life care are related to chronic disease, palliative care and, increasingly, aged care.

The recent string of major child sexual assault scandals, in Australia and other countries, can create a feeling of disgust and an urge to look away from an ugly reality. Yet we must confront and take collective responsibility for child protection by acknowledging that it happens every day and that we have to talk about it. Societal silence on child sexual abuse protects perpetrators and enables abuse to continue. Child sexual assault is a lot more common than we may think. This longitudinal study assesses the attainment and development of children followed from the age of 3 until the end of Key Stage 1 (age 8). Over 700 children were recruited to the study during 1998 and 1999 from 80 pre-school centres in Northern Ireland. Both qualitative and quantitative methods are used to explore the effects of pre-school experience on children's cognitive attainment and social/behavioural development at entry to school and any continuing effects on such outcomes up to 8 years of age.
