BACKGROUND: Approximately 50% of pregnancies in the United States are unintended, leading to negative health, psychosocial, and financial outcomes. Supported methods for preventing unintended pregnancies are promoting education and access to highly effective contraceptive methods, including long-acting reversible contraceptives (LARCs). Studies demonstrate counselor knowledge and bias contribute to underutilization of LARCs, highlighting the need for standardized education.

LOCAL PROBLEM: The setting for this practice improvement project was a rural obstetrical unit in Tennessee where there was no standardized postpartum contraceptive education. Rates of contraceptive use at postpartum discharge were low. The purpose of this project was to implement …

ABSTRACT

BACKGROUND: Adequate sleep is found to improve overall physical and mental health. Unfortunately, hospitalized children receive significantly less sleep than what is recommended by the National Sleep Foundation.

LOCAL PROBLEM: Patient and family feedback at the project site demonstrated inadequate sleep and a noisy hospital environment. The objective of this EBP project is to improve sleep for the hospitalized pediatric patient.

Objective

To improve cultural competency levels of registered nurses on the Mother/Baby unit by educating nurses.

Background

Demographics are shifting in the U.S. with an increase in minority populations. Research has revealed insufficient education or a complete absence of education, resulting in nurses that are not equipped to adequately care for culturally diverse patients.

Methods

The Evidence-Based Practice Improvement (EBPI) Model guided the development and implementation of the project. Neonatal opioid withdrawal syndrome (NOWS) is related to opioid exposure in utero, and cases of NOWS have been increasing. In Tennessee in 2020, 824 newborns were born with NOWS. NOWS treatment involves pharmacological and nonpharmacological treatment. Breastfeeding, a nonpharmacological treatment, decreases the need and length for pharmacological treatment, the length of stay, and NOWS symptoms. Skin-to-skin may also reduce symptoms of NOWS. Common ethical principles are the foundation for why we ask parents for informed consent before proceeding with their infant's care. Respecting parental decisions in periviable resuscitation can benefit from adopting a new practice. This evidence-based practice project detailed the identified needs-based practice change for a standardized perinatal/neonatal outpatient consult concerning the periviable infant. Initiating early consultation of high-risk deliveries provides an understanding of parental wishes and satisfaction in decision-making, care of their infant, and bereavement support through a shared decision-making model. BACKGROUND: Current statistics indicate cesarean section rates in nulliparous term singleton vertex (NTSV) pregnancies, a singleton first pregnancy > 37 weeks’ gestation in a vertex position, of 32.8% and rising. A peanut ball, a tool used to optimize pelvic opening and fetal head rotation, can decrease labor length and cesarean section rates. The aim of this evidence-based practice improvement project was to reduce the cesarean rate in NTSV pregnancies by 1% within three months.
