Abstract: Surgical site infections (SSI) make a significant global contribution to morbidity, mortality, and cost while remaining one of the most preventable causes of healthcare-associated infection. Perioperative antibiotics are a mainstay of prevention, but antibiotics are also associated with cost, risk, and increasing resistance. Dermatology is responsible for more oral antibiotic prescriptions than any other discipline. Despite a trend toward conservative prescribing practices and antibiotic stewardship in dermatology overall, antibiotic prescriptions in dermatologic surgery continue to increase, with a notable rise in short-term perioperative prescribing. Background: Medical repatriation is the practice of returning patients to their country of origin when they have major medical problems_. Forced repatriation, or “medical deportation”, disproportionately affects undocumented individuals. This project aimed to assess the prevalence of medical repatriation in Vermont, identity factors that lead to it, and recommendations to prevent it.

Methods: Interviews were conducted with ten stakeholders including physicians, lawmakers, and advocates for undocumented patients. Team members then analyzed interview transcripts for major themes.

Introduction:

Evidence-based protocols (EBP) exist to guide clinicians in decision-making, however, EBP are often delayed and not optimally implemented [1]. Antibiotic stewardship is heavily guided by EBP and highly relevant to surgical practice. Antibiotic regimens for one of the most common surgical diseases, acute appendicitis (AA) can be highly variable. Post-operative antibiotic (POA) use in non-perforated AA has been largely shown to be non-beneficial and potentially harmful [2]; 4 days of POA in cases where source control is obtained has shown to be non-inferior to commonly prescribed longer courses [3]. The University of Vermont Medical Center (UVMMC) Critical Care Transport (CCT) team received a grant in 2020 to trial one Community Paramedic to reduce high volume utilizers of UVMMC’s ED enrolled in Vermont’s accountable care organization (ACO), OneCareVT. That summer, this team helped develop and analyze the implications of that program.

The current literature surrounding Community Paramedicine was explored, focusing particularly on studies that analyzed costs savings of these programs, rural areas with similar populations as Vermont, and the average cost of an ED visit. The reviewed literature and annotations are included in this work.

Partnership between a health municipality in Kosova, in the former Republic of Yugoslavia and Dartmouth Medical School, New Hampshire, to rebuild the primary care system after the war. The project, which included nurses from both countries, is described, focusing on the changing roles of nurses.
