Nursing Education For Central Line-Associated Bloodstream Infections In Intensive Care Units, Carren Sandra Mullings

Central line associated blood stream infections (CLABSIs) are the most acquired hospital infection in the United States. CLABSI is caused by bacteria that enter the bloodstream through a central line. In the United States, CLABSI has a mortality rate of 18%, and every year 250,000 people are infected. Of these, 30,000 patients die due to CLABSI infections, hospital stays are prolonged, and hospital costs are increased. Patients in an intensive care unit (ICU) are more exposed to CLABSI because of procedures that break tissues and skin, such as the insertion of a central line. A gap in practice was identified …

Over the past three decades, opioid medication misuse and abuse has skyrocketed. The increase in improper use has created the need for more frequent exercise of ethical reasoning skills in practice. This study was designed to determine the effect of an ethics-centered debriefing exercise following a standardized simulation scenario concerning opioid misuse/abuse on nursing students’ value of and perceived confidence in ethical reasoning skills. 18 senior level BSN students at James Madison University in Harrisonburg, VA, participated in an ethics- focused debriefing exercise following the simulated scenario. Advanced directives (AD) allow an individual to retain autonomy in end-of-life decisions and can prevent unnecessary costs and treatments associated with aggressive life-prolonging measures. As of 2017, only an estimated 36.7% of the United States adult population had a form of advance directive documented (Yadav et al., 2017). The purpose of this evidence-based project was to determine if implementation of an advanced care planning (ACP) protocol would increase ACP discussions, AD completion, and result in a modification in stage of change or behavior. Nursing is an ethical profession in which nurses are called to act ethically, be moral agents and function with moral integrity. Contemporary nursing practice is morally pluralistic and at times, nurses may be faced with a conflict of conscience that motivates them to voice their ethical concerns about patient care provision that they perceive to be unethical. This concern can result in the format of a conscientious objection. Conscientious objection involves an individual objecting to doing something they deem unethical and to refrain from participating in that unethical action.

Moral distress is a key issue in the healthcare work environment. This course will explore situations in which health care providers may find themselves that result in moral distress; situations can arise from patients, their families, co-workers, or the organization. Providing quality, evidence based practice is many times limited to doing what is allowed per protocols or payors, and not always what is best for that given situation. Included in this presentation are ways to affirm what is felt, assess sources of distress, contemplate risks and benefits of action, and prepare for action.

Ethical knowledge and skill is crucial to the discipline of nursing and is considered foundational knowledge for nursing practice (American Nurses Association [ANA], 2008). Nurses who assume roles in clinical teaching may be clinically competent but may have limited nursing education experience or knowledge in clinical instruction. The purpose of this project was to improve the educational experience of clinical instructors in the teaching of ethics to students in the clinical setting.
