Chronic disease states including heart failure (HF) often lead to reduced physical activity, resulting in disuse and low-level systemic inflammation, which contribute to the decline in muscle mass and function. An important goal of therapy in HF patients is to counteract or prevent the development of skeletal muscle alterations in order to restore a normal functional capacity. Background: Limited data exist on the role of chronic hyperglycemia-related cardiac autonomic dysfunction in incident heart failure (HF). We investigated the relations of resting heart rate (HR) and PR interval with chronic hyperglycemia, and effects on new-onset HF.

Methods: We prospectively studied 550 patients with prior coronary disease, ischemic stroke and/ or type 2 diabetes. Incident HF was ascertained. Heart rate-PR interval dissociation was defined as counter-physiological co-occurrence of increased PR interval (>mean; 173.3ms) and increased resting HR (>mean; 64.4 beats/min).

Results: Over 63 ±11 months, 6.7% patients developed new-onset HF. Resting HR³75bpm was independently predictive of new-onset …

Problem Description: Research has shown that health information is difficult for the average adult to understand while clear communication helps patients feel involved and increases adherence to treatment. The Joint Commission recommends Health Literacy (HL) assessment and the use of the Teach Back Method (TBM). Medication errors that occur between the RN and elderly patient are preventable when the RN communicates effectively about the patient’s medications. A national focus for healthcare reform is preventing hospital readmissions. Thirty-day unplanned hospital readmissions impact patient outcomes and are costly to the healthcare system. This project explored the impact between the discharge navigator and 30-day unplanned readmissions for heart failure and sepsis populations in a 238-bed community hospital located in central Virginia. Aim: To evaluate if continuing the education of Congest Heart Failure patients post-discharge will decrease the amount of readmissions within 6 months of discharge.

Background: Causes for decreased readmission rates in Congestive Heart Failure patients have been evaluated in multiple studies. The evaluation of the current research showed having discharge education and post- discharge follow-ups decreased the rate of readmission within 6 months. There is a sufficient amount of evidence supporting the implementation of education upon discharge and follow-ups of Congestive Heart Failure patients.

The purpose of this capstone project was to better understand Registered Nurses' knowledge level of commonly taught heart failure education topics. Heart failure is a large reason for hospital readmissions, and subsequently, a major contributor to rising health care costs. Research for this project was completed at a Regional Midwestern hospital using a questionnaire consisting of 20 knowledge questions. Registered nurses working on three separate units with high volumes of heart failure patients were invited to participate. Sixty-nine nurses responded to the survey and obtained an average score of 16.67 out of a possible 20, or 83%. Nurses work in …
