The Federal Government enacted the Heath Information Technology for Economic and Clinical Health Act (HITECH) in 2009, which incentivized providers to use electronic health records (EHR) for meaningful use (MU); the Patient-Centered Medical Home (PCMH) stems from the Act. Research by the National Council on Quality Assurance (NCQA) shows that primary care practices have a positive influence on the health of patients, families and communities. These positive influences are related to their ability to deliver first-contact access, and to develop long-term therapeutic relationships that focus on the person rather than the disease, improving patient health outcomes and reducing health care …

Hypertension is a common primary care problem with guidelines to help with management, however recent studies have shown there may be important considerations in blood pressure management. Blood pressure varies during different times of day, and one summarized study showed significant benefits to taking blood pressure medications at bedtime on major cardiovascular events. In addition, individual variation in blood pressure may also pose a difference in stroke risk and certain medications address this variation better than others. A systematic review and meta-analysis are summarized that address these issues.

Background
Postpartum anxiety and depression has detrimental effects on the overall mental development of children. This study aims to assess the impact of postpartum anxiety and depression on children’s mental development on all sub-scales in a Pakistani population.
Methods
A quasi-experimental study was conducted in two peri-urban communities of Karachi, a mega city of Pakistan, to assess the impact of postpartum anxiety and depression on children’s growth and mental development. A total of 420 women were enrolled, who had given consent out of 651 pregnant women identified, during February 2004 to December 2005. Data for socio-demographic, home environment and family …

Extract: A growing frustration in clinical medicine is that we are now so busy managing the proliferation of risk factors, “incidentalomas,” and the worried well that we lack the time to care properly for those who are seriously ill. As the definitions of common conditions such as diabetes and kidney disease have expanded and the categories and boundaries of mental disorders have grown, our time and attention for the most worryingly ill, disturbed, and vulnerable patients has shrunk. Too much medicine is harming both the sick and well.

Background: The management of medical conditions is influenced by whether clinicians regard them as "disease" or "not a disease". The aim of the survey was to determine how medical students classify a range of conditions they might encounter in their professional lives and whether a different name for a condition would influence their decision in the categorisation of the condition as a 'disease' or 'not a disease'. Methods: We surveyed 3 concurrent years of medical students to classify 36 candidate conditions into "disease" and "non-disease". Prescription drug abuse among adolescents is a growing and increasingly lethal problem. Recent survey studies show a concerning increase in abuse and poor awareness among users, parents, and medical providers. Access to prescription drugs of abuse is often through relatives’ supply of medications. Categories of abused drugs include opiates, stimulants, and benzodiazepines. Methamphetamine is a special case as prescription-only pseudoephedrine can drastically change patterns of abuse. Awareness programs are emerging for youths, parents, community members, and medical providers; but data on effectiveness of these interventions is not yet available.

BACKGROUND: Overuse of unnecessary medications in frail older adults with limited life expectancy remains an understudied challenge.
OBJECTIVE: To identify intervention studies that reduced use of unnecessary medications in frail older adults. A secondary goal was to identify and review studies focusing on patients approaching end of life. We examined criteria for identifying unnecessary medications, intervention processes for medication reduction, and intervention effectiveness.

BACKGROUND: It is unclear whether the higher burden from colorectal cancer among blacks is due to an increased biological susceptibility. OBJECTIVE: To determine whether non-Hispanic blacks (blacks) have a higher risk of adenoma recurrence than non-Hispanic whites (whites) after removal of colorectal adenoma. DESIGN: Secondary analysis of the Polyp Prevention Trial (PPT) data. SETTING: United States. PATIENTS: Patients were 1668 self-identified whites and 153 blacks who completed the 4-year trial. Of these, 688 whites and 55 blacks enrolled in a posttrial, passive Polyp Prevention Trial Continued Follow-up Study (PPT-CFS) and underwent another colonoscopy. - Randomized control trials show beta-blockers benefit patients with systolic congestive heart failure and for the first three years after a patient has a MI.

Introduction: Water and sanitation interventions were delivered in the northern areas of Pakistan as a joint venture of the Aga Khan University and the Aga Khan Health Systems Oshikhandass Diarrhea and Dysentery Project (1989-96) followed by the Aga Khan Water, Sanitation, Health and Hygiene Studies Program (WSHHSP). Through these interventions water treatment plants, new pit latrines along with a component of health education were introduced. Objectives: To explore perceptions, knowledge and practices of inhabitants of Oshikhandass village in Gilgit related to water quality, latrine use and hand washing following the intervention.
