Acute poisoning accounts for 10-20% of acute medical admis- 5ion5 in the UK. There is wide geographical variation in the substances involved in poisoning. In the UK, paracetamol poisoning is common, but it accounts for only 7% of overdoses in the USA. Poisoning with tricyclic antidepressants (TeAs), selective serotonin re-uptake inhibitors (SSRls) and drugs of misuse is very common in the UK and USA. In Southeast Asio, pesticide ingestion is endemic, and constitutes the most common cause of death by poisoning_

Chloroquine is used to prevent and treat malaria and to manage systemic lupus erythematosus and rheumatoid arthritis. It is the most severe and frequent cause of poisoning by antimalarial drugs. Among 167 chloroquine poisoning cases admitted to a toxicology critical care unit, the mortality was less than 10%; these are the best survival figures quoted in the literature to date. The worldwide consumption of alcohol is increasing. For uniformity, the consumption of alcohol often is expressed in terms of units of ethanol ingested. Table 14-1 shows how units of alcohol routinely are calculated. Not everyone who abuses alcohol develops liver damage, however. It is estimated that at least 80% of heavy drinkers show some features of fatty liver, 10% to 35% develop alcoholic hepatitis, and 10% develop cirrhosis. A retrospective study in men showed that 50% of men with an average intake of alcohol of greater than 160 g (15 to 16 units) per day for 20 years developed cirrhosis. …

Migraine and epilepsy are episodic disorders that share many clinical features and underlying pathophysiological mechanisms. Cortical spreading depression (CSD), a wave of profound cellular depolarization, is believed to underlie migraine aura and to be a trigger for the headache pain in migraine. However, the initial event preceding CSD is cellular hyperexcitability associated with localized epileptiform discharges. Glutamate is a critical mediator of the hyperexcitability in both focal seizures and migraine. In focal epilepsy, seizure generation and spread is mediated by synaptically released glutamate acting on AMPA receptors, whereas triggering of CSD depends on NMDA receptors and spread does not require …
