The New York State Dental Journal

Adenoid ameloblastoma with dentinoid (AAD) is a rare benign neoplasm of odontogenic epithelium, with under 50 cases having been reported in the literature. AAD shares histopathological characteristics with both ameloblastomas and adenomatoid odontogenic tumors. They are unique because of dentinoid deposition without enamel. We present a case of AAD in the maxilla of a 13 year old. The patient was treated with left infrastructure maxillectomy and left partial infratemporal fossa dissection. This case reports on an extremely rare disease process in an uncommon location in a pediatric patient.

Use Of Dentoalveolar And Zygomatic Implants In Restoration Of Maxillary Defect Following Fibula Free-Flap Reconstruction Failure, Paul Canallatos Dds, Ms, Thom Loree Md, Amanda Colebeck Dds, Ms, Maureen Sullivan Dds

The New York State Dental Journal

Oral cancer resections cause tissue defects which result in functional complications such as masticatory dysfunction, dysphagia, articulation and phonation issues. When the defect involves the maxillary arch, obturation is needed whether it be surgical or prosthetic. In cases where surgical procedures become ineffective, prosthetic options could be challenging. This is a case of a 78-year-old male developed the diagnosis of squamous cell carcinoma of the right alveolar ridge. The patient underwent partial maxillectomy and right neck dissection with fibula free-flap reconstruction of the maxillary arch. Despite a positive doppler reading, the fibula free-flap became non-vital seven days post-operatively. At the …

Flapless Approach To Dental Implant Surgery: To Use Or Not To Use?, Rebecca Skaf Dds, Kenneth Parrera Dds, Abhiram Maddi Dds, Msc, Phd

The New York State Dental Journal

Flapless approach to implant surgery is emerging as a viable alternative to traditional implant surgery, where a full thickness flap is raised and the alveolar bone is exposed for placement of dental implants. However, there are several disadvantages of the traditional approach including patient morbidity, bleeding and post-operative complications. With the advent of digital technologies such as cone beam computed tomography (CBCT) and 3D printing of surgical guides, flapless surgery has become desirable for implant placement, especially for general dentists. This manuscript discusses the benefits and risks of the flapless approach as compared to the traditional approach for dental implant …

This article addresses a rare postoperative complication of persistent bleeding and “liver clot” coagulum following placement of a permanent tooth pre-formed metal crown, an extraction of a carious permanent molar, and an accelerated exfoliating primary molar in an individual with medical compromise, all in young patients. Mild bleeding following dental procedures or tooth exfoliation is not an unusual occurrence; however, persistent bleeding is an uncommon event for patients. A review of literature revealed 15 reports of liver clots following adult dental procedures, but no documented cases of liver clot formation in young patients. The purpose of this article was to …

The aim of the present study is to determine the prevalence and demographics of temporomandibular joint (TMJ) condylar changes in the asymptomatic general population of a hospital based Oral and Maxillofacial Surgery (OMFS) practice. We retrospectively reviewed cone beam computed tomography (CBCT) scans of 272 TMJs from 136 consecutive patients to detect osteophyte, subchondral cyst, sclerosis, cortical erosions, and flattening. While gender did not appear to play a significant role in TMJ pathology, age was positively correlated with prevalence of condylar changes. Patients 78 years and older were significantly more likely to have osteophyte, cyst, sclerosis, and breakdown than were …

The New York State Dental Journal

People living with HIV/AIDS (PLWH) have an increased life expectancy due to advances in antiretroviral therapy. In 1996, the life expectancy for a 20-year old living with HIV was 39-years old; today, it is >70-years old. As life expectancy increases, PLWH are burdened with age-associated comorbidities. This paper reviews the interaction between aging and HIV and how it affects social, physical, and oral health issues. The shift in demographics presents new challenges for clinicians in how to identify, address, and manage these complex manifestations. This review summarizes how to approach HIV-related changes when providing care as a dental clinician.

Osteomas are frequently reported in the maxillofacial region with much lower incidences in the maxillary and sphenoid sinuses. An unerupted third molar within the maxillary sinus coexisting with a maxillary sinus osteoma is an extremely rare pathologic finding. Such cases can be managed endoscopically or intraorally through a Caldwell-Luc approach. The following case report deals with a patient who presents with an uncommon pathological finding: an osteoma embedding an ectopic tooth within the maxillary sinus.

A Dentist's Role In The Delivery Of Culturally Competent Oral Health Care To Transgender Patients, Laura Rhein Dds, Benjamin Solomowitz Dmd, Michael Mand Dds

The New York State Dental Journal

The importance of delivering patient-centered care is understood by professionals across all healthcare fields. But to deliver such patient-centered care involves understanding various patient populations, especially marginalized populations. Rendering non-biased, non-prejudicial care to marginalized populations is irrefutably critical. To do so, it requires understanding the specific needs of these at-risk populations to deliver high quality patient-centered care. The transgender community is one marginalized population that faces inequities in healthcare. This article seeks to examine current literature to reveal such inequities that the transgender population encounters in dental care and to explore the dentist’s role in the delivery of culturally competent …
