INTRODUCTION: As the population continues to age there is expected to be an increase in prevalence of femoral neck fractures. The standard tool for guiding treatment decisions in femoral neck fractures is the Garden Classification, which is based around the severity of displacement. Garden I and II fractures display minimal displacement and suggest internal fixation as the preferred surgical option rather than arthroplasty.

INTRODUCTION:

Corticosteroids are the mainstay treatment for cardiac sarcoidosis (CS), which manifests clinically in about 5% of sarcoid patients. Current comparisons of steroid regimens in CS focus on starting dose and rarely describe taper rates or expected cumulative exposure. This study characterizes several prednisone regimens for CS to strengthen frameworks for defining treatment goals.

METHODS:

We queried a registry of 1,403 patients referred to the Hospital of the University of Pennsylvania for positron emission tomography (PET) investigation of CS. A gentleman in his 60’s presented to the hospital with dyspnea, fever, and hypoxia. He was diagnosed with Covid-19 pneumonia and treated with BiPAP, oxygen, dexamethasone, enoxaparin, budesonide, and albuterol. His hospital stay lasted 33 days. He was discharged home on oxygen and steroids. He returned to the hospital 3 days later with altered mental status. He was diagnosed with HSV encephalitis and treated with 2 weeks of acyclovir. Although his HSV resolved, he suffered temporal lobe necrosis and lasting neurologic dysfunction.

Introduction: Compartment syndrome is a well known phenomenon that is most commonly reported in the extremities. However, paralumbar compartment syndrome is rarely described in available literature. The authors present a case of paralumbar compartment syndrome after high intensity deadlifting.

Methods: The patient is a 53 year old male who was diagnosed with paralumbar compartment syndrome one day after performing high intensity deadlifting and subsequently treated emergently with fasciotomies. The patient’s medical records were thoroughly reviewed at the presenting institution. Introduction: The effects of delayed surgical repair for traumatic hip fractures have been widely discussed in the literature, and there exists a strong association between surgical timing and patient outcomes. This study is designed to analyze experiences of the PCOM Philadelphia community regarding COVID19 vaccination. While previous studies have examined specific adverse reactions to this treatment, the present survey allowed participants to indicate which vaccine they received (Pfizer-BioNTech, Moderna mRNA-1273, or J&J/Janssen) and how many doses, as well as demographic information, underlying health conditions, prior COVID infection and symptoms, and any reaction to the COVID vaccination. Cancer metastasis to the bone distal to the knee or elbow, also known as acrometastasis, is a rare but known finding in diffuse malignancy. We report a case of acrometastasis to the left second phalanx in a 70-year-old woman with a history of invasive breast carcinoma that was in remission for four years. On initial encounter in the emergency department, she had pain and swelling over the proximal (PIP) and distal interphalangeal (DIP) joints with a lytic appearance on radiographs. Bariatric surgery is used as a treatment for morbid obesity and often results in rapid weight loss. This procedure has been associated with postoperative nutritional deficiencies. Neurological complications due to nutritional deficiencies include Wernicke encephalopathy, a disorder that affects the central and peripheral nervous system due to thiamine (vitamin B1) deficiency. Wernicke encephalopathy can lead to irreversible consequences if not treated early. Background: HSV infections can characteristically present as pseudotumor in patients with uncontrolled HIV infection. Herpes simplex virus type 2 (HSV-2) infection rates range from 60% to 90% in individuals who are co-infected with HIV.

Case Presentation: A 48-year-old patient who presented with large fungating mass near her right inferior vulva with hardness and induration. The mass was 4 cm x 3 cm in size and was excised in the operating room. The pathology was negative for malignancy, however, it showed lymphoplasmacytic proliferation with immunostaining positive for HSV virus.

Bladder diverticulum, which can be present in 1% to 10% of the general population, is an outpouching of the bladder urothelium through the muscular bladder wall. The vast majority of bladder diverticula are acquired and occur more commonly in men with neurogenic dysfunction or bladder outlet obstruction. Due to the lack of a muscular layer, bladder diverticulum loses its contractility and ability for effective urine emptying, which increases the risks of chronic inflammation and malignant transformation. AAs are typically located in the pelvis and perineum and occur more frequently in females of reproductive age. We report histopathological features of concurrent malignant and benign nephrosclerosis in an 83-year-old elderly cadaver. Upon routine dissection in a medical school cadaveric anatomy lab, the abdomen of an 83-year-old female showed contracted and granular bilateral kidneys, with subsequent measurements identifying decreased size. Tissue samples were retrieved from the left anterior descending artery, left ventricle wall, bilateral kidneys and lungs. Histopathology slides were prepared using hematoxylin and eosin (H&E) stains. We present a case with an atypical variation of the ulnar artery discovered while examining a cadaver in our routine Gross Anatomy lab dissection. A superficial ulnar artery was found in a 55-year-old male cadaver originating directly from the brachial artery. This is a rare variation where the route of the ulnar artery runs superficially to the flexor compartment in the forearm. Based on previous cadaver studies, the incidence of such variation in the adult population appears to be approximately 3.75%. During routine dissection of 10 cadavers that originated from the Body Donor Program at Philadelphia College of Osteopathic Medicine South Georgia, an 82-year-old female with a Baker's cyst anomaly located medially in the right popliteal fossa was encountered. The Baker's cyst contained synovial fluid in a thick bursal covering. This finding is suggestive of pathology in the knee joint, such as arthritis or a tear in cartilage or ligaments. This case study analyzes the Baker's cyst found during dissection and its clinical significance.

During a routine lab dissection, lesions were found to be scattered along the mesentery of the colon and small intestines, as well as the abdominal peritoneum. Due to its presentation and histological analysis, a diagnosis of adenocarcinoma was made. Adenocarcinomas are mucus-secreting cells that are found within and near the glands of organ (Nakhaie, Mohsen et al., 2020). Introduction

Hypersensitivity pneumonitis is a rare interstitial disorder characterized by an immune reaction of the lung parenchyma, usually to a sensitized allergen. Pneumonitis is a common cause of respiratory decline among those with comorbid conditions such as chronic obstructive pulmonary disease and congestive heart failure. The use of corticosteroids in patients with pneumonitis is well documented, but their role in the treatment is not clearly understood.

Multisystem inflammatory syndrome in children (MIS-C) is a severe hyperinflammatory condition characterized by fever, systemic inflammation, and multiorgan dysfunction that manifests late in the course of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). MIS-C generally occurs 2 to 6 weeks after SARS-CoV-2 infection. The incidence of MIS-C is unknown, although it appears to occur in less than 1 percent of children with confirmed SARS-CoV-2 infection. The cerebellum, known for its role in coordination and motor adaptation, is hypothesized to travel through the thalamus in its signaling to and from the cerebral cortex, specifically through the ventral medial (VM) and ventral lateral (VL) thalamic nuclei. The thalamus is located within the diencephalon between the midbrain and cerebral cortex. It is an important relay center between the cerebellum and the motor cortex.
