Abington Jefferson Health Papers
BACKGROUND: Management of chronic total occlusions (CTO) in diabetics is challenging, with a recent trend towards early revascularization [ER: Percutaneous coronary intervention (PCI) and bypass grafting] instead of optimal medical therapy (OMT). We hypothesize that ER improves morbidity and mortality outcomes in diabetic patients with CTOs as compared to OMT.
AIM: To determine the long term clinical outcomes and to compare morbidity and mortality between OMT and ER in diabetic patients with CTOs.
METHODS: Potentially relevant published clinical trials were identified in Medline, Embase, chemical abstracts and Biosis (from start of the databases till date) and pooled hazard ratios (HR) …

Ophthalmology and Visual Science Faculty Publications
CONTEXT: Diabetic retinopathy (DR) is the leading cause of incident blindness among working-age adults in the United States. Federally designated safety-net clinics (FDSC) often serve as point-of-contact for patients least likely to receive recommended DR screenings, creating opportunity for targeted interventions to increase screening access and compliance.
STUDY DESIGN AND METHODS: With such a goal, we implemented and assessed the longitudinal performance of an FDSC-based telemedicine DR screening (TDRS) network of 22 clinical sites providing nonmydriatic fundus photography with remote interpretation and reporting. Retrospective analysis of patient encounters between February 2014 and January 2019 was performed to assess rates of …

Objective The objective of this study was to evaluate the association between osteoporosis and diabetic peripheral neuropathy in patients with type 2 diabetes mellitus (T2DM). Background Diabetic complications and osteoporotic fractures are two of the most important causes of morbidity and mortality in older patients. There is increased fracture rate associated with T2DM, despite these patients having greater bone mineral density. Osteocalcin, one of the bone turnover markers, is a peptide secreted by bone cells and reflects bone formation and consequently indicates bone remodeling status; which is the major mechanism underlying osteoporosis. Patients and methods This case–control study was done …

There are two types of diabetes mellitus, Type 1 which involves a decrease in insulin production and Type 2 which is related to an impaired sensitivity to insulin due to peripheral insulin resistance (Chesworth, 2019). Type 1 diabetes is a dysfunction of the pancreas that an individual is usually born with and affects about 10 percent of the diabetic population. Type 2 diabetes is a product of obesity and age that affect the body’s sensitivity to insulin, not a problem with its production, and affects the remaining 90 percent of the diabetic population (Chesworth, 2019). The hyperglycemia that comes with …

Diabetes mellitus is the 7th leading cause of death in the United States, comprising about 10% of the population. With intermittent fasting gaining popularity in the media as a method to lose weight and control chronic illness, patients with diabetes would likely turn to any of the fasting methods in order to better manage their disease. However, there is a paucity of information to support whether intermittent fasting is an effective method to control diabetes, in conjunction with lifestyle modifications and anti-diabetic pharmacotherapy. This review sought to identify clinically relevant human studies that provide evidence that intermittent fasting is …

Necrotizing fasciitis is a rapidly destructive affliction of soft tissues, with a mortality rate that may reach 73% of the cases. It is characterized by a progressive inflammation and extended necrosis of the subcutaneous tissue and the fascia. Necrotizing fasciitis was first described in 1848, and later in 1920 Meleney identified 20 patients in China in which the infection was presumably triggered by hemolytic streptococcus, linking pathological bacteria to the condition. In 1952, Wilson coined the term necrotizing fasciitis although without successfully identifying the specific pathological bacteria involved. In most cases, both risk and aggravating factors are present, the main …

The Thai Journal of Veterinary Medicine
An 8-month-old intact male Thai Bangkaew bred dog presented with polyuria, polydipsia, polyphagia, exercise intolerance and weight loss over 2 weeks. The serum biochemistry profiles revealed hyperglycemia, glycosuria and high serum fructosamine concentrations indicating the development of diabetes mellitus. The dog had a good response to a half-dose of intermediate-insulin injection twice daily monitored by the improvement of clinical signs and serum fructosamine. This was supposed to be due to the partial destruction of islets of Langerhans. Two months later, voluminous diarrhea with greasy stools, dry coat hair and generalized alopecia were observed. However, the clinical signs of diabetes mellitus, …

Genetic variants that are associated with susceptibility to type 2 diabetes (T2D) are important for identification of individuals at risk and can provide insights into the molecular basis of disease. Analysis of T2D in domestic animals provides both the opportunity to improve veterinary management and breeding programs as well as to identify novel T2D risk genes. Australian-bred Burmese (ABB) cats have a 4-fold increased incidence of type 2 diabetes (T2D) compared to Burmese cats bred in the United States. This is likely attributable to a genetic founder effect. We investigated this …

Systematic Review Of Loss-Frame Messaging And Change In Health Behavior, Eddie Jean Simmons
Walden Dissertations and Doctoral Studies
Recent analysis estimates Type II diabetes affects 23.4 million people in the US. Approximately 30% of these patients are not in optimal control of their diabetes, raising risks for blindness, amputation, and cardiac disease. A gap in diabetes education practice is that traditional positive-framed messaging has not persuaded many patients to modify their self-management behaviors. There is a need for alternative ways to communicate the risks of not managing the disease to patients who may have different health beliefs. The purpose of this systematic review was to examine the best available evidence regarding the efficacy of loss-frame messaging when used …

Abstract
BACKGROUND: Diabetes mellitus (DM) is the leading cause of new blindness in American adults. As the number of people with type 2 diabetes continues to increase, the total number of people affected by diabetic retinopathy (DR) will continue to rise. Improved access to screening for DR, followed by treatment, if necessary, can reduce the progression to vision loss. Despite national recommendations, less than half of Americans with DM complete annual diabetic retinopathy screenings (DRS).
PURPOSE: The purpose of this project was to improve assessment, education, documentation, and ordering of DRS for patients with a diagnosis of type 1 and/or …
