Concepts Of Illness Among The Swahili Of Lamu, Kenya, Rebecca Gearhart, Munib Abdulrehman

The Swahili of Lamu, Kenya, understand illness as the result of a spiritual imbalance caused by personal transgression or an attack by harmful forces directed by an envious person. Another underlying component of the Swahili concept of illness is that each person’s physical body operates in conjunction with personal attributes that are fixed at birth and determine moral character, behavior, and predisposition to ailments. When physical symptoms occur, the Swahili focus on identifying the human or supernatural entity that caused the illness in consultation with a range of healers who specialize in a variety of curing strategies. We sought to determine the feasibility of measuring heart rate turbulence (HRT) derived from electrocardiographic (ECG) bedside monitors in critically ill respiratory failure patients, receiving mechanical ventilation. In addition, we aimed to determine whether or not a normal HRT response was associated with patients' successful return to spontaneous breathing, also called ventilator weaning, and survival to discharge.

The training available to perioperative nurses in rural areas is often inefficient and can be difficult to access. We designed a distance training programme using educational material from the Association of Perioperative Registered Nurses (AORN). Over a six-year period, we compared the performance of 22 nurses undertaking the programme via telemedicine with that of 13 nurses undertaking it conventionally, in person. The telemedicine nurses were based at six rural hospitals and interacted with their preceptors via remotely controlled videoconferencing systems. Interest in simulation as a teaching and evaluation strategy in nursing education continues to grow. Mirroring this growth, we have seen a proliferation of instruments designed to evaluate simulation participant performance. This article describes two frameworks for categorizing simulation evaluation strategies and provides a review of recent simulation evaluation instruments. The review focuses on four instruments that have been used extensively in the literature, objective structured clinical examinations (OSCE's) including four OSCE instruments, and an extensive list of new instruments for simulation evaluation.

Drawing on Connell’s (Gender and power: Society, the person and sexual politics. California: Stanford University Press, 1987; Masculinities. Berkeley, CA: University of California Press, 1995) model of gender relations, this paper examines patterns of intimate partner violence among women who have recently left an abusive partner. In so doing, we attempt to better understand the social structural factors that shape the relations of power and control in intimate violent heterosexual unions. Although the negative health effects of intimate partner violence (IPV) are well documented, little is known about the mechanisms or determinants of health outcomes for women who had left their abusive partners. Using data collected from a community sample of 309 Canadian women who left an abusive partner, we examined whether women's personal, social and economic resources mediate the relationships between the severity of past IPV and current health using structural equation modelling. Current text-based discharge instructions are unsuitable for low-literate older adults who have difficulty following written action-based instructions. To address this issue, we developed discharge instructions for low-literate older adults after hip replacement surgery using pictographs (i.e., simple line drawings with stick figures showing explicit care actions). The aim of this focus group study was to examine the acceptability and comprehension of these instructions to 15 low-literate older adults. Participants perceived that the pictograph-based discharge instructions helped them understand the intended health care messages, especially for step-by-step procedures of discharge actions.
