Diffuse idiopathic skeletal hyperostosis (DISH) is a common disorder in middle-aged and elderly patients but often ignored. It is characterized by calcification and ossification of the spinal ligaments, as well as entheseal ossification within extraspinal sites. The etiology of DISH is still unknown. It can result in chronic pain and stiffness and occasionally severe disability. It often mimiks with spondylosis and ankylosing spondylitis. We report a 71-year-old male patient who suffered from chronic back and shoulders pain for many years. DISH was diagnosed after a series of examinations.
