Clinical Multimedia: Ultrasound Examination Of Medial Meniscus Posterior Root Tear, Yu-Xuan Wu, Chien-Ping Chen, Hung-Jui Chuang
Rehabilitation Practice and Science
This report outlines the case of a 52-year-old woman with continuous left knee pain. An initial assessment, suggesting a meniscal tear based on joint line tenderness and a positive McMurray test, led to an ultrasonographic investigation. The ultrasound revealed left medial meniscus extrusion without positional change from full extension to 90° flexion, a hypoechoic cleft in the medial meniscus posterior root (MMPR) area, and the notable absence of the MMPR anterior to the posterior cruciate ligament (PCL), identified as the ultrasonographic ghost sign. A subsequent MRI confirmed a medial meniscus posterior root tear (MMPRT). MMPRT is known to cause biomechanical …

Ultrasonography Can Be Early Diagnostic Modality For Pediatric Calcaneus Fractures: A Case Report, Ying-Chu Chen, Szu-En Chen
Rehabilitation Practice and Science
Although calcaneus fractures may be the most common pediatric tarsal bone fracture, most occult calcaneus fractures are not visible on plain radiography, resulting in delayed diagnosis. However, ultrasonography can aid early diagnosis of this fracture with an unspecified mechanism of injury and unremarkable radiographic findings. In a pediatric patient with progressive ankle swelling and pain, initial plain radiography was unremarkable, and antibiotics were administered for suspected infective arthritis. After limping developed, ultrasonography showed symmetric growth plates of bilateral distal tibia, fibula, and tarsal bone; a step-off calcaneus cortex with periosteal reaction; increased power Doppler flow; and probe tenderness, indicating a …

Entrapment Of The Deep Branch Of The Radial Nerve By An Intramuscular Lipoma: A Case Report, Yann-Rong Chen, Ying-Chen Kuo, Yu-Hsien Lin, Yu-Ting Lin
Rehabilitation Practice and Science
Lipomas are benign tumors that are common in subcutaneous tissue and are usually asymptomatic. However, intramuscular lipomas, which are less common, can cause nerve compression in rare cases. Herein, we report the case of a 61-year-old man with an intramuscular lipoma on his right forearm. The lipoma entrapped the deep branch of the radial nerve and caused wrist drop and finger drop. A nerve conduction examination and electromyography were conducted to quantify the degree of and localize the nerve injury. Sonography provided the initial diagnosis of a lipoma, which was verified with magnetic resonance imaging. The lipoma was resected, and …

Age-Related Sonoanatomical Change Of The Knee In Children, Shu-Mei Yang, Chia-Wei Lin, Wen-Shiang Chen
Rehabilitation Practice and Science
Ultrasound has been thoroughly studied and become a well-established tool for assessment of adult patients' musculoskeletal system both in research and clinical practice. However, the use of musculoskeletal ultrasound in pediatric populations is still limited. The age-dependent variability of normal sonoanatomy dueto bone maturation and ossification in children is the main limitation to establishing its validity as an imaging tool. Also, standardized definitions of gray-scale and power Doppler ultrasonography findings in different age groups of children is still lacking. Nonetheless, ultrasound is a readily accessible imaging modality that is well tolerated by children. It also allows physicians to perform a …

The anatomical structure of the neck area is exceedingly complex; thus, cervical nerve damage during surgical procedures is not uncommon. A 25-year-old woman underwent excisional biopsy for neck lymphadenopathy. The patient reported left upper back pain and left shoulder weakness 1 day after the surgery. Physical examination revealed weakness of the left upper trapezius muscle and winging of the left scapula. A nerve conduction study indicated decreased compound muscle action potential amplitude on stimulation of the left spinal accessory nerve (SAN); electromyography also revealed denervation changes with the presence of voluntary motor unit action potentials in the left upper trapezius …

Periarticular Dextrose Prolotherapy For Patients With Primary Knee Osteoarthritis: A Casereport And Literature Reviewarticle, Yen-Jou Ho, Yao-Jen Chen, Chi-Chung Ho, Sheng-Wei Zeng, Su-Ju Tsai, Long-Wei Dai
Rehabilitation Practice and Science
Knee osteoarthritis (OA) is a common disease that primarily afflicts older adults. The mainstream management of knee OA involves integrative practice, including patient education, weight reduction, exercise, pharmacological therapy, and non-pharmacological modalities. However, prolotherapy with hypertonic dextrose via either intra-articular or periarticular injection, or a combination of both techniques, has become a strong evidence-based treatment or adjuvant therapy that may be used in addition to conservative management. In the literature, two studies have compared the effects of intra-articular and periarticular prolotherapy for knee OA and reveal comparable effects. In addition, the periarticular approach has fewer side effects and post-injection pain. …

Myxoid Liposarcoma: A Soft-Tissue Sarcoma Mimicking Hematoma In A Baseball Player, Chia-Hua Lee, Hao-Hung Tsai, Yat-Yin Law, Chuan-Chao Lin
Rehabilitation Practice and Science
There are multiple histological types of soft-tissue sarcomas (STS) that are associated with different prognoses. Liposarcoma (LPS) is the most frequent subtype of STS in extremities and trunk wall in Taiwan. The myxoid liposarcoma is the second most common subtype of LPS and is associated with an intermediate-grade malignancy. STS and hematoma may be difficult to differentiate clinically. We report a case of a 35-year-old male presented with a mass lesion over the left thigh, which was initially misinterpreted as a hematoma on ultrasound. We also conducted a magnetic resonance imaging (MRI) study, which revealed the possible diagnosis of STS. After …

Plantar Fasciopathy At Unusual Location: A Report Of Two Cases, Chia-Hua Lee, Feng-Cheng Tang, Yao-Jen Chen, Long-Wei Dai, Su-Ju Tsai, Chi-Chung Ho
Rehabilitation Practice and Science
Plantar fasciopathy is a common problem in the general population involving the proximal area of the central cord of the plantar aponeurosis. Few studies have discussed plantar fasciopathy distal to the calcaneus. In article reviewarticles, plantar fasciopathy distal to the calcaneus occurred in only 12% of the cases. In the casereport, including two cases in which fasciopathy were involved in the central part of the plantar fascia, the incidence was obviously less than that mentioned above. These two cases were atypical diseases of plantar fasciopathy over the plantar arch. They both received one to several sessions of prolotherapy, but no …

Rehabilitation Practice and Science
Ultrasound is increasingly used for the diagnosis of musculoskeletal conditions, particularly in soft tissue injuries. In some cases, it is useful for the evaluation of fractures. A 54-year-old woman presented with focal a non-traumatic right-sided chest pain. Physical examination revealed a focal area of tenderness on palpation. Erect chest X-ray was unremarkable. A focused musculoskeletal ultrasound was carried out to examine the area of tenderness and a rib fracture which was not detected on the erect chest X-ray was diagnosed during sonographic examination. A brief literature reviewarticle on the use of ultrasound in the diagnosis of fracture, in particular rib …

Ultrasonography In The Diagnosis Of Carpal Tunnel Syndrome And Measurement Of Gliding Of The Median Nerve, Yi-Wei Chang, Yi-Chieh Lien, Yi-Shiung Horng
Rehabilitation Practice and Science
The main purpose of the diagnostic ultrasound examination for carpal tunnel syndrome (CTS) is to compensate for the inadequacies of physical examinations and nerve conduction studies and thereby reduce the false negative rate of CTS diagnosis. The cross-sectional area of the median nerve, swelling ratio, flattening ratio, and palmar bowing of the flexor retinaculum have been reported as the diagnostic criteria for CTS in ultrasonographic examinations. In addition, Doppler ultrasound has been applied to measure the blood flow of the median nerve, and dynamic ultrasound combined with a cross-correlation algorithm has been developed to measure the gliding of the median …

Rehabilitation Practice and Science
A 38-year-old man with uncontrolled type 2 diabetes mellitus complained of a mild painful swelling over the right posterior thigh that had been present for 3 days. He was initially diagnosed as having a right hamstring strain. After a 7-day treatment with non-steroidal anti-inflammatory drugs, the symptoms progressed to a prominent painful swelling over the right posterior thigh. Leukocytosis and high inflammation indices were noted. The ultrasonography of the right posterior thigh showed an ill-defined heteroechoic lesion surrounded by an irregularly thick hyperechoic and hypervascular wall in the muscle layer. The diagnosis was pyomyositis of the right biceps femoris muscle, …

Rehabilitation Practice and Science
Plugged ducts are a common problem during lactation. They often occur when the breast is not emptied properly, or when there is local pressure on the breast. Plugged ducts usually occur three to four weeks postpartum. Symptoms include a sensitive lump with well-defined margins in the breast. There may be pain and tenderness in the affected area, although this is not necessarily accompanied by systemic symptoms such as high-grade fever. Milk flow can be blocked and sometimes a tiny white spot can be seen at the opening of the mammary duct on the nipple. Without treatment, plugged ducts can lead …

Rehabilitation Practice and Science
High-resolution ultrasonography is a useful tool for the localization of non-palpable soft-tissue foreign bodies. A 21-year-old female underwent a routine health examination, during which chest radiography revealed a radio-opaque linear foreign body. The foreign body was located within left posterior chest wall, 4 to 5 cm lateral to the T8 spinous process. The shape indicated a needle. The possible presence of a needle in the chest wall meant that an operation to remove it was imperative. There was no tender point or palpable mass present on the chest wall and, consequently, there was difficulty determining the precise location of the …

A 31-year-old lady complained of multiple painless masses on her left arm. Two masses were clearly visible in the left palm and distal forearm, and a smaller lump was also palpable in the left antecubital area. A neurogenic tumor in the left palm was diagnosed by magnetic resonance image (MRI). Sonography demonstrated highly vascular neurilemmomas along the median nerve in the palm, distal forearm, and antecubital area. A nerve conduction velocity study of the median nerve showed prolonged distal latency of the motor nerve portion but normal sensory function. The tumors were resected, and pathology examination confirmed the diagnosis of …
