The anatomical structure of the neck area is exceedingly complex; thus, cervical nerve damage during surgical procedures is not uncommon. A 25-year-old woman underwent excisional biopsy for neck lymphadenopathy. The patient reported left upper back pain and left shoulder weakness 1 day after the surgery. Physical examination revealed weakness of the left upper trapezius muscle and winging of the left scapula. A nerve conduction study indicated decreased compound muscle action potential amplitude on stimulation of the left spinal accessory nerve (SAN); electromyography also revealed denervation changes with the presence of voluntary motor unit action potentials in the left upper trapezius …

Tibial Neuropathy After Revision Total Knee Prosthesis: A Casereport, Chun-Ping Wang, Wei-Ting Wu, Lin-Fen Hsieh

We describe a 74-year-old woman with right knee osteoarthritis experiencing right tibial neuropathy because of complications of total knee arthroplasty revision. Weakness in the right toe flexors and paresthesia over the sole of the right foot were observed after operation. A nerve conduction study and electromyographic examination indicated right tibial neuropathy. Radiography indicated a prosthesis fragment in the right popliteal fossa, and its location was different before and after surgery. Prosthesis fragment dislodgement was also confirmed through ultrasonography, and the nearby tibial nerve may have been entrapped because of the dislocation process and surrounding tissue fibrosis. Compressive Femoral Neuropathy Caused By Iliopsoas Muscle Hematoma: A Casereport, Szu-Hung Tsao, Yu-Ting Hsieh, Tai-Yi Wu, Shu-Hua Lin

Femoral neuropathy is uncommon compared with other mononeuropathies. We report a case of femoral neuropathy in a 53-year-old man who has cardiovascular disease and took Coumadin for 8 years. He has left lower extremity pain and weakness because of femoral nerve and lumbar plexus compression caused by an iliopsoas hematoma, which resulted from a period of prolonged bleeding. The patient’s condition improved after surgery and rehabilitation. We also reviewarticle the anatomy and pathophysiology of the femoral nerve, the clinical presentation of femoral neuropathy, the diagnostic tools used, the treatment method, and the prognosis.

Needle electromyography (EMG) is an important diagnostic and prognostic tool in physical medicine. However, some patients are unable to tolerate the pain associated with the procedure. While there has been research in methods aimed at reducing the pain from needle EMG, all have obvious disadvantages.TENS is a widely accepted tool for producing analgesia, but it has not been studied as an analgesic adjunct to needle EMG. Close links between the bilateral trapezii are essential for the stability and coordination of neck and shoulder movements. However, there is no direct neurophysiological evidence to support such a bilateral motor control mechanism. Our study is the first to use an electrodiagnostic method to prove the existence of this reflex. In this study, we have identified a new uniform or standardized technique, named the trapezius reflex (TR), which is able to evaluate proximal shoulder motor function. This study has the following purposes: (1) to inquire the incidence of radiculopathy on the asymptomatic side in the cervical spondylotic patients with unilateral symptomatic radiculopathy. (2) to inquire the relationship between asymptomatic side and symptomatic side in the unilateral symptomatic radiculopathic patients due to cervical spondylosis. All subjects has been studied by motor and sensory nerve conduction test, and electromyography (EMG) for muscles supplied by roots from C-5 to T-l levels bilaterally.
