Purpose: Adaptation to psychosocial aspects of hand injury is often undervalued, yet a critical issue to address in traumatic hand injuries (Schier & Chan, 2007). The purpose of this scholarly project was to develop a guide to address such factors in a hand therapy setting.

Methodology: A review of the literature was conducted to demonstrate the importance of hands, psychosocial implications related to hand trauma, and current strategies used by practitioners to address these problem. Decreased adherence to therapy protocols after upper extremity injury has been correlated with increased risk of re-injury and decreased general therapy outcomes. Occupation-based therapy has been shown to increase adherence to therapy protocol. To date, there have been several occupation-based protocols in upper-extremity musculoskeletal rehabilitation, but never one that addresses occupation across the lifespan and attempts to develop a tool to assist therapist in developing occupation based protocols specific to the developmental group.

According to Leatherwood (1997), approximately 30% of all emergency room injuries are injuries of the upper extremity, amounting to 16 million hand injuries per year seen in the United States. According to the Arthritis Foundation (2008), an estimated 46 million adults in the United States (21 %) report having doctor diagnosed arthritis. Traumatic injuries and cumulative trauma such as arthritis are two of the diagnostic categories most commonly treated by hand therapists in the United States and Canada (Muenzen et aI., 2002). In the field of hand therapy there are a variety of protocols for treatment of patients with tendon injury. Review other literature indicates that most of these protocols have a good anatomical and physiological basis, which then requires the application of clinical experience to decide which protocols to use when treating patients with hand injury.
