To date, most research on massive multiplayer online role-playing games (MMORPGs) has focused on the effects playing MMORPGs has on players’ health and wellness. The virtual environment of MMORPGs has yet to be explored as a space where meaningful occupations occur. This qualitative study explored the virtual environment using a transactional perspective in order to define the qualities of virtual worlds, how transactional relationships operate more clearly within these virtual worlds to construct occupational meaning, and subsequently to provide a foundation of qualitative data to support future research including virtual worlds. Occupational therapy providers working with young children and their families in early intervention settings frequently provide interventions to support family routines related to social participation, health management, and related occupations at home and in the community. Family routines are inextricably tied to individual and family health and well-being, yet contextual influences can impact satisfaction with and performance of routines resulting in disparities in health and occupational outcomes. Current research on children with Fragile X Syndrome (FXS) lacks inclusion of qualitative outcomes on the child’s daily occupational performance. Standardized measurements are frequently utilized and provide useful information, however, can be less sensitive to change (Berry Kravis et al., 2013) and miss capturing family perspectives and improvements within meaningful occupations. This research incorporates family perspectives via semi-structured interviews to promote an in-depth understanding about FXS and its impact on child and family occupations in addition to standardized assessment scores through in-depth case study analysis. Purpose: In 2016, there were 22.5 million refugees worldwide (UNHCR, 2017). California resettled just over 5,000 of those 85,000 (Igielnik & Krogstad, 2017).
