Objective: Identify associated factors with emergence delirium in children in the Post-Anesthetic Recovery Unit of a Pediatric Health Specialized Institute in Peru. Methods: A cross-sectional study was carried out in children from 2 to 6 years old, physical status classification I and II, in elective procedures under anesthesia. The main variable was the presence of emergence delirium evaluated with the Pediatric Anesthesia Emergence Delirium scale greater than or equal to 10 points. To identify the factors associated with delirium, the Poisson regression model was applied and a p value < 0.05 was considered significant. Results: Of the 150 children, it was found that the incidence of emergence delirium occurred in 10.6%. Of these, 81.4% were under 5 years of age, pain was significantly associated in 37.5% with PR = 3.63, 95%CI [1.20, 10.1] and it was observed that 68, 8% required palliative care. Conclusion: Pain was associated with delirium of postanesthetic emergence in children. Prevention and treatment of emergence delirium should focus on postoperative pain control and careful monitoring of patients after surgery.

Introduction: Hospitals of greater complexity tend to care for patients with advanced chronic diseases, which is why it is important to recognize the need for palliative care. Objective: To identify the proportion of patients who require palliative care in the medicine department of a Peruvian referral hospital. Methods: Observational, analytical, cross-sectional study. The quality of life perceived by patients with advanced cancer from a tertiary hospital is compared according to home care versus hospital care. Observational, cross-sectional study, included patients older than 18 years with cancer with multiple metastases. EORTC QLQ-C15-PAL quality of life survey was applied to patients with adequate cognitive level (Pfeiffer questionnaire). Chi square and Mann-Whitney, p <0.05, and SPSS 24.0 were used. 83 patients in the hospital group and 69 at home. Median age 66 and 76 years respectively (p <0.01); the female sex predominated (p = 0.60). They presented moderate or severe cognitive impairment, 25% hospital group and 58% home (p <0.01). The median quality of life score was 49.5 in the hospital group and 48 in the home group (p = 0.60), the physical functional aspect had a lower score in the hospital group (p <0.01). The perceived quality of life is acceptable, the home group is older and has cognitive impairment.

This paper argues the value of systems thinking to patients, family members and medical practitioners in end-of-life care, particularly as a mechanism for considering when palliative care should be introduced as preferred treatment. It applies a well-established set of tenets in systems thinking retrospectively to a case study of patient care in Australia. This highlights how and where different decisions might have been made, based on a holistic consideration of the patient’s best interests.
