The development and validation of 30-day readmission risk prediction models, such as the LACE index, have been of interest to researchers and healthcare organizations, especially since the Centers for Medicare and Medicaid Services began to impose monetary penalties on hospitals with higher than expected 30-day readmission rates. However, there is a lack of consensus concerning the efficacy of the LACE index in the heart failure population. Heart failure patients are responsible for managing fluctuations in symptoms between exacerbations by employing treatment adherence, active monitoring, and management strategies based on expert guidelines. Despite education, delayed help seeking persists among those in the need of acute medical intervention, as evidenced by high hospital admission and readmission rates. The purpose of this qualitative grounded theory study was to explore the decision making processes undertaken by heart failure, community-dwelling individuals as they experience symptom changes. Cognitive impairment is commonly seen in the elderly population. It is unclear if cognitive deficit in heart failure (HF) patients is a primary factor for higher hospital readmission rates in this population. The Centers for Medicare and Medicaid Services have established strict guidelines for reimbursement on readmissions that occur within 30 days. It is imperative that organizations identify and rectify issues that impact readmissions.
