The Government of India in 2014 signaled its commitment to adolescent health through introduction of a national strategy, Rashtriya Kishor Swasthya Karyakram (RKSK). Targeting adolescents aged 10–14 and 15–19, the program aims to ensure universal coverage of health information and services for all adolescents—those in and out of school, married or unmarried, and in vulnerable groups. Envisaged as a paradigm shift to address adolescent health beyond sexual and reproductive health, RKSK spans six domains: nutrition, sexual and reproductive health, mental health, injuries and violence including gender-based violence, substance misuse, and non-communicable diseases. This program brief describes a Population Council initiative in three low-income areas of Ethiopia. Kalkidan (Amharic for “Promise”) is a three-year program designed to increase discussion and information-sharing on HIV and related topics between marital partners; to promote HIV prevention and health service utilization within marriage, including prevention of mother-to-child transmission (PMTCT) of HIV; and to build husbands’ support of their wives, as well as investment in their health and that of their children. Kalkidan recruits and trains community-based mentors to lead parallel discussion groups of husbands and wives, who periodically come together for joint sessions. The Population Council is …

Is Early Childhood Care And Education A Good Investment For Egypt? Estimates Of Educational Impacts, Costs, And Benefits [Arabic], Caroline Krafft

This summary details the impact of early childhood education in Egypt. The results show that early childhood care and education has an impact on educational attainment that is both statistically significant and sizeable. Such interventions increase overall educational attainment by approximately one year, primarily due to decreased primary and preparatory drop out. The impact on educational attainment is reflected in improved school performance, such as higher test scores, decreased grade repetition, and improvements in school tracking, during the early years. Investments in early childhood education can be a powerful approach to improving educational outcomes, and the research suggests that increases …

Challenges Facing The Egyptian Education System: Access, Quality, And Inequality [Arabic], Caroline Krafft

This brief analyzes and summarizes young people's responses to the 2009 Survey of Young People in Egypt (SYPE) to identify the greatest challenges facing the Egyptian educational system, focusing specifically on primary through secondary schooling. Results show that access to school has improved, but some youth, especially females in rural Upper Egypt, remain outside the school system and are increasingly marginalized. The Egyptian school system is delivering low-quality education that is irrelevant to the labor market and has problems with repetition, absenteeism, and drop out which reduce the efficiency of the education system. Using a combination of qualitative and quantitative data, this working paper investigates whether access to improved sources of water and sanitation is an effective “treatment” for the incidence of diarrhea among children under five years of age in Egypt. Both components of the study indicate that widespread access to improved sources of drinking water and toilet facilities exists across Egypt; however, service quality remains a significant problem in many areas. In particular, cuts in water supply—and the resulting practice of storing water—are quite common; a sizable percentage of flush toilet facilities are not connected to the public sewer system; and …

In the past decade, there have been expanding resources to address the underlying gender dimensions of HIV. This has been particularly urgent in sub-Saharan Africa as the female-to-male infection ratios in young populations has reached 3 to 1 and sometimes above. The phrase “gender and HIV” has become commonplace yet does not provide any specific guidance as to target audiences, content, or measurable results. It can include everything from microcredit programs for HIV-positive women, to workplace programs seeking to change negative male norms, to efforts to increase respect for diverse sexual and gender identities. This report is a midterm evaluation and documentation of the process and approach of the FGM-Free Village Model implemented in Egypt by the National Council of Childhood and Motherhood with assistance from the Population Council. In a comparison of responses from women and men in intervention groups to those in nonintervention (control) villages, data analysis shows that the program has been successful in changing views and attitudes toward female genital mutilation (FGM). FGM is an entrenched generational practice, and eradicating it in a community requires concerted effort over an extended period of time. This evaluation strongly recommends that efforts be …

This brief provides details about the Biruh Tesfa (“Bright Future”) program in Ethiopia established by the former Ethiopia Ministry of Youth and Sports (currently Ministry of Women, Children and Youth) with technical assistance from the Population Council. Biruh Tesfa aims to address the social isolation of adolescent girls by building their social capital and participation and giving them access to basic literacy, life skills, and HIV and reproductive health information. Looking at the current employment situation of youth in six states in India, this Population Council policy brief argues that significant investment in terms of appropriate policies and programs is required to improve the employability of youth in the country and thereby enable them to find full and productive employment. Concerted efforts to improve the employability of youth and to enhance the reach of existing programs aimed at job creation among youth are most urgently required in states like Bihar, Jharkhand, and Rajasthan. At the same time, efforts to meet the large unmet need for vocational skills training are needed …

Looking at the current educational and employment situation of youth in Andhra Pradesh, this Population Council policy brief argues that significant investments in terms of appropriate policies and programs are required to enable the state to achieve the Millennium Development Goals of universal primary education and elimination of gender disparity in primary and secondary education, harness its demographic dividend, and enable its youth to participate in and benefit from global development. Notwithstanding the state’s existing policy commitments, findings indicate that several challenges remain. The brief recommends a number of policies and programs to overcome these challenges.

Findings from six states call for actions to promote youth participation, particularly the participation of young women, in civil society and political life. Programs are needed at the school, college, and community levels—through national service programs, sports, and other nonformal mechanisms—that encourage civic participation, incorporate value-building components, and reinforce secular attitudes and values that espouse responsible citizenship. This policy brief describes the Population Council’s project to improve access and use of demographic, health, and other public sector data that are essential evidence for development policy planning and evaluation. In many countries, access to data is limited and, when data are available, the format is rarely user-friendly for nonprofessionals, limiting their potential applications. In recent years, the Population Council has examined access to and sharing of data through case studies in Ethiopia, Ghana, Senegal, and Uganda. In phase II of the project, the Council will strive to promote domestic demand for data by working with key intermediaries—the media.

To better understand the lives of adolescents in rural Ethiopia, researchers conducted a survey of adolescents in Amhara Region. The study found that many girls experienced early, unwanted arranged marriages, resulting in early unwanted sexual initiation and pregnancy, as well as social isolation in new marital homes. Based on the findings of this study, the Population Council and the Ethiopian Ministry of Youth and Sports created a program to support girls who are married early, with a view to increasing their social networks, improving their ability to protect themselves from HIV/AIDS, and supporting their reproductive health. This working paper reviews the evolution of the HIV/AIDS epidemic and provides estimates of past trends and future projections of AIDS mortality indicators, including numbers of AIDS deaths, the proportion of all deaths that are due to AIDS, and life expectancy. A cumulative total of 24 million people have died from AIDS between 1980 and 2007, and by 2030 this total is projected to reach 75 million. Despite the rapid spread of this new disease during the 1980s and 1990s, the epidemic has reached a major turning point in recent years as the rate of new infections peaked and began …

The recent introduction of user fees for Vietnam’s primary healthcare services has generated concern that economic policies may adversely affect health-seeking behavior and health outcomes of the poor, particularly among impoverished families who are members of socially marginalized minority groups. This Population Council working paper examines this debate by analyzing parental recall of illness and care-seeking for sick children under age 5 recorded by the 2001–02 Vietnam National Health Survey. Ethnic differentials are evident in all geographic and income levels, although adverse effects of minority status are most pronounced among poor households in remote areas. This issue of Quality/Calidad/Qualité describes the evolution of Profamilia through its work on gender-based violence in the Domincan Republic.Their project was conceived along two simultaneous paths: providing support services directly to women and girls who had experienced violence and initiating advocacy in the wider policy arena. Profamilia joined the commission that ultimately designed and promoted a law to increase protection against violence, especially domestic violence against women and children. Although the clinics now run a dynamic service program, the agency has also sustained its advocacy activities. The Effect Of Community Nurses And Health Volunteers On Child Mortality: The Navrongo Community Health And Family Planning Project, Brian Wells Pence, Philomena Nyarko, James F. Phillips, Cornelius Debpuur

This report presents the child mortality impact of a trial of primary health-care service-delivery strategies in rural Ghana. After adjustment for sociodemographic factors, under-five mortality in areas with village-based community-nurse services fell by 16 percent during the five years of program implementation compared with mortality before the intervention. Reductions were observed in infant (6 percent), early child (20 percent), and late child (41 percent) mortality. Community involvement and training of a local health volunteer were associated with an 11 percent increase in mortality, primarily driven by a 124 percent increase in early child mortality. This edition of Quality/Calidad/Qualité discusses the Manuela Ramos Movement and its efforts to improve women’s health and well-being through a range of empowerment strategies in rural Peru. A joint project with USAID, named Reprosalud, demonstrates how combining the resources of an international donor with local women’s organizations allows a more organic and multifaceted family planning program to develop. Such programs can produce impressive improvements on a number of indicators, including contraceptive use.

The 1994 International Conference on Population and Development in Cairo was a watershed moment in the definition of population policies. The meeting put an end to the unproductive debate on which is more instrumental in achieving voluntary fertility decline: providing family planning (FP) services or improving social and economic development. The answer was that both are essential. The Cairo meeting also defined the most desirable services and the kind of development that was most empowering, particularly with respect to achieving reproductive choice. The Hanover Street Project, formally known as the United Women’s Woodworking and Welding Project, is an experiment in training women for jobs usually held only by men. Begun in 1976, this was the first such program of the Jamaica Women's Bureau, established by the government during International Women's Year to ensure that women participate fully in Jamaica’s development. The project demonstrated that low-income women can learn non-traditional skills and can work together to improve their lives. Through trial and error, the project is providing the Women's Bureau with a wealth of information about teaching technical skills, working with other government …
