Development And Testing Of A Mapcheck3-Based Patient-Specific Qa Method For Prime Intensity Modulators Used With Bolus Electron Conformal Therapy, James T. Crist

Purpose: Irregular surfaces in bolus electron conformal therapy (BECT) create target volume dose inhomogeneity, which off-axis intensity modulation (IM) can reduce. This research developed and tested a patient-specific clinical quality assurance (QA) procedure for fabricated Passive Radiotherapy Intensity Modulation for Electrons (PRIME) devices used for IM-BECT delivery.
Methods: To evaluate MapCheck3 (MC3) diode array accuracy, electron dose measurements beneath PRIME devices were compared with scanned measurements in a water phantom. Lateral dose profile comparisons were made for 35 combinations of five beam energies (7-20MeV) and seven PRIME devices - converging and diverging island blocks (pins) having 100-68.8% intensities. In vitro multicellular spheroids are attractive model systems for assessing genetic and epigenetic changes that occur in diseased tissues. Understanding how such alterations in gene and subsequent protein expression affect disease progression and metastasis, drug resistance, and recurrence is of great interest in cancer research. In this regard, examining expression and activity of proteins, such as those with cytoprotective ability that are overexpressed in cancer cells, in addition to cell phenotype (i.e., stem-like, epithelial, mesenchymal, or mixed), are two ways to evaluate genetic and epigenetic changes. The work presented here has 3 goals: establish the need for novel neurostimulation waveform solutions through a literature review, develop a neurostimulation pulse generator, and verify the operation of the device for neurostimulation applications.
The literature review discusses the importance of stimulation waveforms on the outcomes of neurostimulation, and proposes new directions for neurostimulation research that would help in improving the reproducibility and comparability between studies.

Challenges in drug efficacy occur during the treatment of most types of cancer due to the heterogeneity of the tumor microenvironment. This has led to the development of personalized medicine. Due to the clinical success of the proteasome inhibitors Bortezomib and Carfilzomib in treatment of multiple myeloma, interest has shifted towards molecularly-targeted chemotherapeutics for ubiquitin-proteasome system (UPS). Deubiquitinating enzymes (DUBs) are an essential part of this pathway which have been found to promote Bortezomib resistance in multiple myeloma patients. Abstract
Purpose: The accurate prediction of stray neutron dose has become increasingly important as it increases the risk of second cancer development after proton therapy. Previously reported analytical models predicted the quantity dose equivalent, which includes physical and biological considerations but does not explicitly take into account material dependence and variation in the radiation quality. The purpose of this study was to investigate the feasibility of an analytical model of absorbed dose to water from stray neutrons in proton therapy.
