Endodontic Therapy, Crown Amputation, And Soft Tissue Advancement To Treat A Hopeless Tooth, Timothy Daws Dds, Thomas Bolten Dds, Md

Managing patients with a history of intravenous bisphosphonates and antiresorptive medications poses challenges in clinical decision-making. Extractions are generally discouraged in these patients due to the risk of medication-related osteonecrosis of the jaw (MRONJ). This case study presents an 89-year-old female with metastatic breast cancer on Xgeva, facing a non-restorable tooth (#30). A collaborative approach involving the general dentist, endodontist, and oral surgeon employed endodontic therapy, crown amputation, and soft tissue advancement, avoiding extraction. The technique, akin to coronectomy, mitigates MRONJ risk by preserving bone integrity. Although there are multiple studies about direct injury to Inferior Alveolar Nerve (IAN) during dental procedures, however, data regarding compression phenomenon on the nerve is limited. The case report aims to review the topic of iatrogenic compression phenomenon of IAN following surgical placement of dental implant and to report a clinical case of compressed IAN with temporary loss of sensation following placement of dental implant in posterior mandible.

Oral cancer resections cause tissue defects which result in functional complications such as masticatory dysfunction, dysphagia, articulation and phonation issues. When the defect involves the maxillary arch, obturation is needed whether it be surgical or prosthetic. In cases where surgical procedures become ineffective, prosthetic options could be challenging. This is a case of a 78-year-old male developed the diagnosis of squamous cell carcinoma of the right alveolar ridge. The patient underwent partial maxillectomy and right neck dissection with fibula free-flap reconstruction of the maxillary arch. The aim of the present study is to determine the prevalence and demographics of temporomandibular joint (TMJ) condylar changes in the asymptomatic general population of a hospital based Oral and Maxillofacial Surgery (OMFS) practice. We retrospectively reviewed cone beam computed tomography (CBCT) scans of 272 TMJs from 136 consecutive patients to detect osteophyte, subchondral cyst, sclerosis, cortical erosions, and flattening. While gender did not appear to play a significant role in TMJ pathology, age was positively correlated with prevalence of condylar changes. Osteomas are frequently reported in the maxillofacial region with much lower incidences in the maxillary and sphenoid sinuses. An unerupted third molar within the maxillary sinus coexisting with a maxillary sinus osteoma is an extremely rare pathologic finding. Such cases can be managed endoscopically or intraorally through a Caldwell-Luc approach. The following case report deals with a patient who presents with an uncommon pathological finding: an osteoma embedding an ectopic tooth within the maxillary sinus.

But to deliver such patient-centered care involves understanding various patient populations, especially marginalized populations. Rendering non-biased, non-prejudicial care to marginalized populations is irrefutably critical. Diagnosis And Management Of Nasopalatine Duct Cyst: A Report Of Two Cases And Literature Review, Feiyi Sun Dds, Harry Dym Dds

The nasopalatine duct cyst (NPDC) is one of the most common non-odontogenic, non-neoplastic developmental cysts. It arises from the epithelial remnants of the nasopalatine duct within the incisive canal and crosses the midline of the anterior maxilla. Most cases are asymptomatic and either are discovered by chance on radiographs or present as a soft tissue swelling, pain, or drainage in the palate. The present study describes the diagnosis and surgical management of two NPDC cases at the Department of Oral and Maxillofacial Surgery at the Brooklyn Hospital Center.
