Medical advances have improved the five-year survival rate for patients diagnosed with cancer and some late-stage cancers that once were terminal have now become manageable, chronic conditions. Because of this, more individuals with cancer as well as survivors of cancer are re-entering the workforce. Resuming the role of worker is considered an important step in recovery for many. Occupational therapy (OT) can play an important role in facilitating a successful transition back to work for both cancer patients and survivors.

Introduction
- In 2016 ASCO recommended that patients with advanced cancer receive dedicated palliative care (PC) services1. Early PC involvement is associated with lower spending, fewer 30-day readmission rates, decreased chemotherapy administration at the end of life (EOL) and increased hospice referrals2.
- Many patients are not referred and continue to receive chemotherapy and utilize high-acuity services near the EOL.
- The Oncology Care Model (OCM) is a CMS episode-based alternative payment model promoting high-value care.
- We evaluated the effect of PC visits on EOL outcomes including code status (CS) and spending in the last 30 days of life.
Intraoperative Opioid Administration Among Cancer Patients, Amanda Barrett, Kenn B Daratha
Intraoperative Opioid Administration Among Cancer Patients, Amanda Barrett, Kenn B Daratha
Books, Presentations, Posters, Etc.
Background Cancer prevalence continues to rise. In vitro and in vivo studies have demonstrated that opioid receptor agonists may be associated with poor outcomes for cancer patients. Laboratory studies have shown that opioids cause immunosuppression; tumor cell progression, recurrence and metastasis. The surgical environment further decreases immune function, causes an inflammatory response and promotes cancer cell mobilization. Anesthesia providers are in a unique position to influence postoperative outcomes. Opioid free anesthetic delivery is a growing trend amongst anesthesia providers and is supported by the literature. The purpose of this project is to compare intraoperative opioid administration rates for patients with …

House Staff Quality Improvement and Patient Safety Conference (2016-2019)
Abstract
Introduction: Breast cancer is not only a physical disease but something that affects our patients’ mental, spiritual, emotional and social wellbeing. There has been literature that demonstrates the positive benefits of exercise, social support, and nonmedical adjuncts for breast cancer patients. There is currently a lack of wellness support within the Jefferson breast cancer community.
Methods: A wellness survey will be distributed throughout the breast surgery and breast oncology clinics to assess the current level of wellness and interest in a breast cancer wellness group at TJUH. This survey will be used both before and after implementation …

A Qi Initiative To Reduce Time To Antibiotics In Oncologic Neutropenic Fever, Adam Binder, Jordan Villars
House Staff Quality Improvement and Patient Safety Conference (2016-2019)
Problem Statement
Neutropenic fever (NF) (defined as temperature of 101o F on one occasion, or 100.4o F sustained over 60 minutes in a patient with an absolute neutrophil count (ANC) less than 500, or suspected drop below 500 within 48 hours) is relatively common oncologic emergency, particularly in hematologic malignancy patients. Expert consensus is that anti-pseudomonas gram-negative antibiotics (abx) should be administered within 60 minutes of detecting neutropenic fever.
At Thomas Jefferson University Hospital (TJUH), internal guidelines for time to treatment in neutropenic fever are in line with expert consensus - 60 minutes. We found that from July 1st – …

Many people pursue direct-to-consumer (DTC) DNA testing to learn about health risks. While DTC testing for breast cancer genes BRCA1 and BRCA2 may provide vital information to those testing positive, it is unclear whether individuals testing negative understand the implications of these results. Furthermore, it is unknown how such findings may influence future cancer risk management. Ninety-six individuals who underwent BRCA testing through the DTC service, 23andMe, completed a survey designed to assess their understanding of a negative result and how this result may impact cancer risk management. Respondents were recruited via social media sources Reddit and Facebook. All participants …

Cancer patients face many obstacles and challenges after being diagnosed with cancer. Thomas Jefferson University Hospitals’ Sidney Kimmel Cancer Center (SKCC) has effectively eliminated transportation as a barrier to care for patients receiving cancer treatments such as chemotherapy and radiation. SKCC dedicates a full-time patient navigator to assess and connect patients with appropriate transportation resources based on location, insurance coverage, and mobility. The utilization of patient-focused transportation platforms, such as Uber Health, RoundTrip, and Ride Health have streamlined the process, while SKCC philanthropic funds have assisted with associated financial costs. As a result of these resources, oncology patients who would …

Pharmacy Posters
Introduction
- Infusion-related reactions (IRRs) are well-known adverse drug reactions of many biological agents and antineoplastic medications
- While typically mild-to-moderate in intensity, IRRs can be severe, with potentially life-threatening consequences requiring urgent interventions
- Appropriate use of pre-treatment medications, such as corticosteroids, antihistamines, intravenous fluids, and antiemetics reduces the incidence and severity of IRRs
Purpose
- Evaluate the use of pre-treatment medications in patients with experienced IRRs at an outpatient infusion center
- Identify opportunities to improve treatment plans and reduce the occurrence of IRRs

• Breast cancer screening imaging options have progressed greatly over • the years in sensitivity, specificity, and image quality. According to DynaMed Plus, in 2012 there were 522,000 deaths by breast cancer and 1,677,000 total cases of breast cancer documented (Dynamed, 2018). For years, traditional screening for breast cancer involved 2D digital mammography which obtains two views of each breast. With advances in technology, the use of 3D breast tomosynthesis has become an advantageous addition to routine breast cancer screening protocols at many health care facilities.

There is discrepancy amongst organizations and the medical community whether bimanual pelvic examinations should be performed in asymptomatic women for routine screening. The purpose of this literature review was to determine whether bimanual pelvic examinations are beneficial for screening for ovarian cancer in comparison to no screening. In addition, research was conducted to see if healthcare providers’ professional beliefs align with the evidence and national recommendations, to determine women’s thoughts and beliefs regarding pelvic examinations, and identify other screening methods if bimanual pelvic exams are determined to be an invalid screening tool. Throughout reviewing peer reviewed articles and high-quality evidence, …

Over the years, oncologists are better able to fight cancer and have increased cancer survival, but we are also finding that these lifesaving therapies can affect the heart and other parts of the body in a negative way. Chemotherapy is excellent at destroying cancer cells, but it also causes collateral damage to other healthy cells. There are certain chemotherapeutic agents that are known to cause cardiotoxicity. Currently we are monitoring the heart function of the patients who are receiving these cardiotoxic drugs prior to starting the chemotherapy as well as during and after treatment. However, there are currently no recommendations …
