The practice problem identified at the project site was the lack of nurses’ knowledge of how cultural preferences influence diabetes self-management practices among African Americans with diabetes mellitus. An educational program focused on the cultural preferences of Sierra Leoneans that influence diabetes self-management practices was provided for 48 nurses who worked in the agency. In 2010, nearly 1 in 5 visits to the emergency department (ED) were categorized as potentially avoidable, resulting in a significant $65 billion increase in health care expenditures and exacerbating ED overcrowding. Inefficient communication between nurses and patients increases the risk of medication errors and jeopardizes patient safety, ultimately resulting in lower patient satisfaction as indicated by Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) scores. The purpose of this quality improvement initiative was to assess the impact of educating nurses on patient medication education using the teach-back method. Data were collected using HCAHPS survey responses from adult patients with medical conditions who had been discharged from a medical-surgical unit. This study involved determining why mothers hesitate to vaccinate their children and where they get their information about vaccinations. This descriptive qualitative study guided by the health belief model explored the influence social media has on mothers’ perceived barriers to vaccinating their children. An educational intervention to improve communication skills was implemented with staff including licensed practical nurses, registered nurses, and nurse practitioners.
