Abstract

Problem: Effective medication communication, including potential side effects, is a key determinant of safe patient care and better health outcomes. 3SA scores a 1-2 out of 5-stars for medication communication with the HCAHPS survey from October 2020-March 2021.

Context: As one of the medical-surgical units at this California hospital, it serves the purpose of providing medical and/or surgical management to acutely ill patients. The unit is a 24-bed, 1:5 nursing ratio with medical-telemetry and Covid-19 overflow.

Intervention: The interventions include a structured, evidence-based multi-component program in improving patient knowledge about new medications they receive, the possible side effects of …

Problem: Unplanned, inpatient surgical patients were experiencing poor outcomes and dissatisfaction with their overall care. This surgical patient population also lacked communication from their healthcare teams with regard to plans of care throughout their hospital stays.

Context: This was a quality improvement project for the unplanned, inpatient surgical patient population in the Central Valley of California. Approximately 13% of this hospital’s surgical patients required post-surgical care in the inpatient units. These patients, according to unfavorable HCAHPS scores, experienced unsatisfying care and insufficient communication from their healthcare teams, including physicians and nurses.

Abstract

Problem: Effective communication about medications and their potential side effects is a significant indicator of safe patient care and improved health outcomes. The Cardiac Procedure Unit (CPU) of a large medical center in California recently received a low HCAHPS (CMS, 2019) score of one star (out of five) in the area of medication communication.

Context: The CPU is a microsystem that provides pre and post cardiac procedure care for patients of diverse backgrounds. It has a highly competent staff administering patient care. Current patient medication education faces challenges that hinder patient receptivity and thus understanding.

The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey has recognized the need for medication education with communication about medication as a patient satisfaction indicator. Literature shows a medication guide or tool that is readily accessible and easy to understand for the patient can help provide nurses with a teach back method to help increase patient knowledge about common medications. Within the explored microsystem, a gap in patient communication was identified through the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) surveys. A mid-sized hospital in Alameda County would like to increase its Hospital Consumer Assessment of Healthcare Providers and System (HCAHPS) scores by improving the patient care experience. The specific aim for this project is that by August 2018, the implementation of daily leadership rounding in the ICU will result in an increase of patient satisfaction HCAHPS scores by 5%. The project team used the Plan-Do-Study-Act cycle to guide the implementation of leadership rounding.
