The purpose of this evidence-based project is to reduce 30-day readmission rates of patients diagnosed with septicemia by using a symptom screening tool in the home health setting. Sepsis is a life-threatening and costly healthcare burden. Through this project the home health department staff were educated on the pathology of sepsis, and the use of the adult sepsis screener to identify at risk patients early and manage their symptoms thereby reducing 30-day hospital readmission rates of patients diagnosed with sepsis.

Obesity and depression share a bidirectional relationship when they exist as comorbidities in addition to individually posing complex public health challenges. Evidence from recent years suggests that there is a benefit to treating these illnesses with an integrated approach when they co-occur, but no current clinical guideline exists for this approach. The purpose of this integrative review is to synthesize existing literature and report current interventions targeting the management of comorbid obesity and depression in adults. Delirium in elderly hospitalized adults continues to be a common and costly issue in health care today. Various delirium protocols and strategies are available to reduce negative outcomes of delirium such as falls, increased length of stay, pressure ulcers, hospital readmissions, the need for transferal to long-term care, and mortality rates. However, many hospitals have still not implemented routine delirium protocols. Because falls in older hospitalized adults are often linked to delirium, this integrative review was undertaken to examine delirium protocols and approaches that specifically decrease falls among older adults in acute care settings.
