Implementation Of An Evidence-Based Toolkit To Improve Colorectal Cancer Screening In A Federally Qualified Health Center, Stephanie Mckithan Mar 2024

Implementation Of An Evidence-Based Toolkit To Improve Colorectal Cancer Screening In A Federally Qualified Health Center, Stephanie Mckithan

Doctoral Dissertations and Projects

The change in payor fee-for-service to value-based care is creating an opportunity for primary care clinics nationally to adjust their focus to meeting key clinical quality measures during patient encounters. Colorectal screening is one of such quality measures. Previous research has shown that early detection saves lives. The guidelines have lowered the recommended age of initial screening to 45 due to the incidence of colorectal cancer in younger patients. But with all the objectives that need to be met in the limited time of the visit, how can nurse practitioners improve their efficiency? This evidence-based project assessed the implementation of …

Provider Knowledge And Confidence With Implementing Pediatric Adverse Childhood Experiences (Aces) Screening Tool In An Alaskan Family Practice Clinic, Lisa Marie Wangberg Aug 2022

Provider Knowledge And Confidence With Implementing Pediatric Adverse Childhood Experiences (Aces) Screening Tool In An Alaskan Family Practice Clinic, Lisa Marie Wangberg

Doctoral Dissertations and Projects

Adverse childhood experiences (ACEs) have been known to be associated with negative developmental and long-term health sequalae since Felitti and colleagues (1998) presented their groundbreaking research nearly a quarter of a century ago. Pediatric and primary care settings have been relatively slow to implement any type of ACEs screening at regular intervals such as at well-child exams (WCEs). A scholarly project implementation was used to present ACEs education and screening training to pediatric primary care providers in a small, private, rural family health care practice in Alaska. The goal of the project was to determine if providers felt more knowledgeable …

The Best Practice For Colon Cancer Screening Recommendations By Primary Healthcare Providers: An Integrative Review, Anna Lynn Viens Jul 2022

The Best Practice For Colon Cancer Screening Recommendations By Primary Healthcare Providers: An Integrative Review, Anna Lynn Viens

Doctoral Dissertations and Projects

Colorectal cancer screening is an important step toward reducing the mortality rate of this disease. Screening rates will improve with the utilization of trusted health care provider recommendations for patients. The purpose for a review of literature on best practices for colon cancer screening recommendations by providers is to increase the number of effective conversations between primary care providers and their patients around colorectal cancer, and, ultimately, to facilitate more provider screening recommendations for patients. This topic is important for reducing the burden of colon cancer. Providers would benefit from education and decision aids to assist with the colon cancer …

Pediatric Primary Care Concussion Evaluation And Management, Kaitlyn Layman Aug 2018

Pediatric Primary Care Concussion Evaluation And Management, Kaitlyn Layman

Doctoral Dissertations and Scholarly Projects

Concussions are an important and timely subject, especially within the pediatric population, as they are the most involved in extracurricular contact sports and are susceptible to concussions and their sequelae. The researcher performed a quasi-experimental pilot study in a pediatric primary care office where the researcher educated 15 providers on the HEADS UP concussion screening tool and management approach. The researcher gave providers pre- and post-intervention surveys to determine if providers’ comfort and knowledge regarding assessing and managing concussions within the pediatric primary care setting changed. Four providers participated in the study. The researcher conducted a chart review two months …

Increasing Advance Care Planning In The Primary Care Setting, Nicole Marie Coffey Aug 2018

Increasing Advance Care Planning In The Primary Care Setting, Nicole Marie Coffey

Doctoral Dissertations and Scholarly Projects

Evidence shows the many benefits of advance care planning (ACP) discussions, including; giving patients peace of mind, decreasing healthcare costs, increasing quality of care, and decreasing distress at the end of life. Due to the many proven benefits of ACP, the United States Department of Health and Human Services as well as many leading national health organizations are promoting ACP, even tying it to reimbursement. The best setting and time to initiate ACP is in the primary care setting as early as possible, before patients have any cognitive decline. In this context, patients have an established relationship with their provider, …

Chart Audit And Educational Provider Feedback Intervention To Improve Appropriate Use Of Spirometry In Patients With Chronic Obstructive Pulmonary Disease, Ruth Demetros Jan 2018

Chart Audit And Educational Provider Feedback Intervention To Improve Appropriate Use Of Spirometry In Patients With Chronic Obstructive Pulmonary Disease, Ruth Demetros

Doctoral Dissertations and Scholarly Projects

Despite being a GOLD guideline and having documented benefits, confirming a COPD diagnosis with spirometry is not routinely done. The purpose of this project was to increase patient referrals for spirometry to confirm COPD diagnosis. A quasi-experimental design was incorporated in a primary care office. A retrospective pre-intervention chart audit and two post-chart audits, 30-days apart, compared the frequency of documented spirometry to confirm a COPD diagnosis. An educational intervention with a pre and post-survey examined provider behavior intention to order spirometry to confirm a COPD diagnosis. The pre-chart audit revealed that 27/50 (54%) of patients had spirometry documented to …
