Background: The non-medical use of opioids has reached epidemic levels nationwide, and rural areas have been particularly affected by increasing rates of overdose mortality as well as increases in the prison population. Individuals with opioid use disorder (OUD) are at increased risk for relapse and overdose upon reentry to the community due to decreased tolerance during incarceration. It is crucial to identify barriers to substance use disorder treatment post-release from prison because treatment can be particularly difficult to access in resource-limited rural Appalachia.

Heroin is an opioid that commonly appears as white or brown powder. Eventually a person can become physically dependent on heroin, meaning the body begins to expect the drug. A sudden withdrawal from the drug can cause intense symptoms such as vomiting, muscle pain, and cravings, often leading to relapse (Shannon, p. 172). Although the initial decision to try heroin may be up to the addict, due to the dependence one develops when using heroin, addiction eventually morphs into an involuntary compulsion. Use Of Psychotropic Medications Among Pregnant Women With Bipolar Disorder: Patterns, Determinants, And Impact On Perinatal Outcomes, Misbah Salim

Navigating psychotropic treatment use for bipolar disorder (BD) in pregnancy is challenging as it is, but it is even more difficult with use of multiple medications across various drug classes for treatment, given the absence of evidence of their safe use. This retrospective cohort study assessed prescription patterns, patient characteristics associated with these patterns, risk of adverse outcomes, and rates of readmission in women with BD who were prescribed antipsychotic, antidepressant, or combination treatment during pregnancy, relative to untreated women. While diagnoses of depression and anxiety are not uncommon for women during pregnancy and after delivery, one of the greatest challenges in the plan of treatment is to ensure that the offspring are kept safe while the psychological well-being of the mother is appropriately cared for. The objective of this literature review is to evaluate the commonly prescribed treatment methods for prenatal and postnatal depression, focusing on the efficacy of antidepressant medications and nonpharmacologic treatments while evaluating the effects these medications have on the fetus/breastfeeding infant. Researchers have suggested that jails and prisons in the United States are becoming the new mental health clinics, contributing to the phenomenon of mass incarceration and costing upwards of $15 billion per year in public revenue. The problem is no conclusive evidence exists that treatment in these custodial environments is more effective than that provided by noncustodial programs; especially for substance users. Additionally, the continuing incarceration of people with mental health problems by the hundreds of thousands poses a difficult ethical dilemma regarding why this population does not receive noncustodial or hospital treatment instead.
