The results of this family medicine resident quality improvement project were: (1) Raised awareness of the importance on proper medication reconciliation processes as tools to increase patient safety, (2) Optimized our medical group medication reconciliation workflow, (3) Implemented steps on the workflow in order to educate patients regarding their medication list, (4) Educated health care providers on a new electronic health record medication reconciliation tool, (5) Compared rate of medication changes during hospital admissions and discharges prior to the intervention and 12 months later, (6) Classified medication changes as justified or unjustified, (7) Classified medication changes according to drug categories. …
