Risk Of Kidney Stones In Living Kidney Donors: A Matched Cohort Study, Sonia Thomas

A kidney stone in a person with one kidney requires urgent attention which may result in surgical and/or hospital attention. We conducted a matched retrospective cohort study to determine if living kidney donors compared to healthy non-donors have a higher risk of: 1) kidney stones with surgical intervention, and 2) hospital encounters for kidney stones. We reviewed and linked information from pre-donation charts to Ontario healthcare databases. We selected healthy non-donors from the general population, matching ten non-donors to every donor, to generate a cohort of 2,019 donors and 20,190 non-donors. Background: Although gestational diabetes mellitus (GDM) has been linked to pediatric obesity, there is limited research on the impact of intrauterine exposure to GDM on trajectories of childhood growth. Objective: To assess the effect of prenatal GDM exposure on childhood body mass index (BMI) trajectories. Design: Analyses were conducted using data from cycles 2 to 6 (1994-2004; N=3412 children) of the National Longitudinal Survey of Children and Youth. Latent growth curve modelling (LGCM) was used to model BMI trajectories from age 2 to 10 years with prenatal exposure to GDM as a predictor. Effect modification by breastfeeding was assessed. The purpose of this thesis was to conduct a process and outcome evaluation of the deterrent effect of Ontario’s Street Racers, Stunt and Aggressive Drivers Legislation. The focus of this study was on police enforcement (implementation), a change in speeding on the provincial highways (intermediate outcome) as well as on a decrease in both extreme speeding convictions and casualties, measured as a sum of injuries and fatalities (criterion outcomes). Epidemiological models structured by time since infection have their origin in the seminal work of 1927 by Kermack and McKendrick. Compared to ordinary differential equations (ODE) models, they are able to capture differences in infectivity of the individuals in a more suitable manner. Background: Several studies show an association between ambient particulate matter (PM) and all-cause mortality. The Adventist Health and Smog 1 (AHSMOG-1) study (N=6,338) has previously found associations between ambient air pollution and incident chronic obstructive pulmonary disease (COPD) using the spatial interpolation method from the three nearest fixed monitoring stations to residence and workplace. A population known for substantial violence is the substance abuse treatment population. This study assessed: (1) extent of violence, (2) personality and mental health correlates of violence, and (3) sex differences in correlates of violence. Data were obtained from the Patterns and Consequences of Cocaine and Alcohol Use for Substance Abuse Treatment Clients Study (N = 403). Logistic regression with backward elimination procedures and modified Poisson regression were used to assess the study objectives. Violence was reported by 44% of treatment clients. Objectives: Certain Canadian subpopulations observe numerous modifiable and non-modifiable risk factors for diabetes. This study compares immigrants and Aboriginals (First Nations, Inuit, and Métis) with Canada-born individuals at higher risks for diabetes, and deciphers the determinant differences between them.

Methods: Pooled Canadian Community Health Survey data (2001-2010) were used. Time trends for diabetes within each subsample were calculated using individual survey year prevalence rates; diabetes diagnoses were self-reported (N=33,565). Various risk factors were also examined using logistic regression.

Lumbar total disc replacement (TDR) surgery is a new procedure that treats degenerative disc disease, a remarkably common, disabling, and costly condition. Three distinct studies were conducted to address the longevity, costs, and geographic variation of the lumbar TDR procedure.

These three studies found lumbar TDR to have a revision burden and economic revision burden that has remained consistent since this procedure’s introduction in the

African, Caribbean, and other Black (ACB) people are a priority group for HIV prevention in Canada, but little is known about the epidemiology of HIV risk in this population. The overall goal of this thesis is to guide HIV prevention interventions for ACB communities. It focuses on social factors that impact HIV vulnerability.

This research used data from the Black, African, and Caribbean Canadian Health Study—a mixed methods study that used 30 semi-structured interviews, and a cross-sectional survey using a structured, self-administered quantitative questionnaire to collect information about HIV and health from 188 ACB people.

Current research shows moderate alcohol consumption is associated with decreased risk of diabetes and excessive consumption or binge drinking can cause insulin resistance and diabetes. In 2010, diabetes was the seventh leading cause of death in the United Statesand was responsible for significant health complications: blindness, kidney failure, and limb amputations, and is a large national economic burden. Campylobacteriosis is a leading cause of gastroenteritis in the United States. The focus of this research was to (i) analyze and predict spatial and temporal patterns and associations for campylobacteriosis risk and (ii) compare the utility of advanced modeling methods. Laboratory-confirmed Campylobacter case data, obtained from the Foodborne Diseases Active Surveillance Network were used in all investigations.

We compared the accuracy of forecasting techniques for campylobacteriosis risk in Minnesota, Oregon and Georgia and found that time series regression, decomposition, and Box-Jenkins Autoregressive Integrated Moving Averages reliably predict monthly risk of infection for campylobacteriosis. Stroke is one of the leading causes of death and disability, with major global public health implications. Stroke ranks No. 4 among all causes of death, behind heart disease, cancer, and chronic lower respiratory disease (CLRD). Stroke accounts for almost 1 of every 18 deaths in the United States. Women accounted for 60.6% of stroke deaths. Death certificate data show that the mean age at stroke death was 79.6 years; males had a younger mean age (76.3) than females.

Cardiovascular Disease (CVD) is the leading cause of death in women and the risk is increased threefold in diabetics. In postmenopausal diabetics, the ovary responds to hyperinsulinemia by increasing secretion of testosterone precursors which increases the risk of CVD. Data from the National Health and Nutrition Examination Survey (1988-2010) and the Study of Osteoporotic Fractures were used to test the hypothesis that a history of bilateral oophorectomy -surgical removal of both ovaries-would decrease the risk of CVD among postmenopausal diabetic women due to reduced androgen levels. Landscape epidemiology, sometimes called spatial epidemiology, is a sub-discipline of medical geography that uses environmental conditions as explanatory variables in the study of disease or other health phenomena. Background: Previous studies have addressed multiple sclerosis (MS) symptom management and improved health-related quality of life (HrQOL). Yet lowered estrogen levels in post-menopasual women with MS may further worsen physical function and symptomology and not all types of pain management have been examined. Objectives: For post-menopausal women with MS, we evaluated the extent to which smoking is associated with worsened health outcomes and HrQOL, and the extent to which menopausal hormone treatment (MHT) improves health outcomes and HrQOL. Introduction: Sexually Transmitted Diseases, including HIV/AIDS, continue to be a major burden in the United States. Sexual partnering behaviors contribute to the spread of STDs. Sexual concurrency has been shown to exponentially increase STD prevalence in populations. Serial monogamy with short periods between sexual partners also introduces risk. Methods: We identified sexually active men and women from the 2006-2010 National Survey of Family Growth (NSFG) and used sub sets for each particular study. Neighborhood food environments have been associated with dietary intake and obesity. Measures of the food environment have typically been characterized with geographic information systems (GIS)-based measures, however, the use of perception-based measures of the food environment have increased in frequency. Despite the benefit of mammography, many women are not up-to-date on screening and do not receive timely follow-up after abnormal mammogram finding. This study compared the association between residential area factors and gonorrhea, aggregated at both the census tract versus ZIP code levels. Prior research has found that census block groupings are most optimal in neighborhood-level studies, followed by census tracts and ZIP codes. Because several healthcare databases currently only provide ZIP code level data, the aim of this study was to examine whether there exists a meaningful difference between the interpretation of census tract and ZIP code aggregate data.
