INTRODUCTION

Frequent hospital utilization is a major contributor to the costs of healthcare. Despite a vast body of literature, there remains great uncertainty about the characteristics and care needs of individuals with frequent hospitalizations.1,2 In the movement to reduce the need for hospitalizations, care management programs have garnered wide attention and support.
