A 2011 Cochrane Review found that adequately randomized trials sometimes revealed larger, sometimes smaller, and often similar effect sizes to inadequately randomized trials. However, they found no average statistically significant difference in effect sizes between the two study types. Yet instead of concluding that adequate randomization had no effect the review authors postulated the “unpredictability paradox”, which states that randomized and non-randomized studies differ, but in an unpredictable direction. However, stipulating the unpredictability paradox is problematic for several reasons: 1) it makes the authors’ conclusion that adequate randomization makes a difference unfalsifiable—if it turned out that adequately randomized trials had …

For many, it seems obvious that food marketing is making us fat. The anger and outrage that was once evoked by tobacco companies is now being repackaged and aimed at ‘Big Food’ (Hennessy, 2014). But is it justified, is food marketing the cause of obesity? And in any case, does outrage and dogmatism help solve the problem? This paper suggests that blaming the marketers, both Big Food and "lazy leisure", is a disputable claim, and dangerously shifts responsibility from individuals to external agents.

Rhetoric scholars (Predergast, Leweicki-Wilson, Pryal) have examined the rhetorical disempowerment of the mentally ill, whose perceived lack of reason isolates them from public discourse. Such isolation can be explained using Kenneth Burke's theory of identification (and its "ironic counterpart," division) which shows how the discursive markers of "sane" and "insane" function to create an in-group, the sane, that relies upon the rhetorical and physical isolation of the insane. The article argues that the mentally ill make an ideal Burkean scapegoat, and that the criminal acts of a few mentally ill people provides the necessary justification for the scapegoating of the …

Background: Suffering a stroke has major implications for the patient. To understand human suffering, one should understand society. Pirandello described society as a higher entity than the individual, thereby justifying human adaptability to society. We explore a qualitative finding that suggests that social trends may influence how stroke patients prioritize aspects of their rehabilitation. Methods: We compare a contemporary patient’s experience of stroke recovery with that of a fictional character from the works of Luigi Pirandello. Both patients had two main residual symptoms: hemiparesis and aphasia. Results: The rehabilitation priorities of the two patients differed, and appeared to reflect the …

This paper utilizes eighteen months of ethnographic and interview research undertaken in 2003 and 2004 as well as follow-up fieldwork from 2005 to 2007 to explore the sociocultural factors affecting the interactions and barriers between U.S. biomedical professionals and their unauthorized Mexican migrant patients. The participants include unauthorized indigenous Triqui migrants along a transnational circuit from the mountains of Oaxaca, Mexico, to central California, to northwest Washington State and the physicians and nurses staffing the clinics serving Triqui people in these locations. Many contemporary scientific researchers are interested in drawing associations between mental illness and creativity. These studies have contributed to the popular image of the "mad genius," an image whose history stretches back as far as Plato and Aristotle. Recently, a new rhetorical manifestation of the mad genius image has emerged, what I call the creativity mystique of mood disorders. The creativity mystique, a product of the era of modern psychiatry, suggests not only that mood disorders are sources of creative genius, but also that medical treatment should take patient creativity into account. The problem-oriented medical record is the widespread, standardized format for presenting and recording information about patients, which is taught to future physicians early in their medical training. Based on our participant obser- vation of medical training, we analyze the ways in which the patient presentation operates in medical training as a disciplinary technology that manages uncertainty in the clinical decision-making process. Every year, the United States employs nearly two million seasonal farm laborers, approximately half of whom are migrants (Rothenberg 1998). This article utilizes one year of participant observation on a berry farm in Washington State to analyze hierarchies of ethnicity and citizenship, structural vulnerability, and health disparities in agriculture in the United States. The farm labor structure is organized along a segregated continuum from US citizen Anglo-American to US citizen Latino, undocumented mestizo Mexican to undocumented indigenous Mexican. The ethnography shows how this structure symbolically reinforces conflations of race with perceptions of civilized and modern subjects. These hierarchies produce what …

From their first colonial experiment in the Canary Islands in 1402, the Spanish administration learned that it was sometimes more effective to import assimilated settlers from already established colonial possessions than to attempt massive conversion and cultural assimilation. When exercising their public health powers, states claim various rights against their subjects and aliens. The paper considers whether public health considerations can help justify some of these rights, and explores some constraints on the justificatory force of public health considerations. I outline two arguments about the moral grounds for states’ rights with regard to public health. The principle of fairness emphasizes that those who benefit from public health measures ought to contribute their fair share in upholding them.
