BACKGROUND: Microvascular dysfunction (MD: impaired performance of blood flow, tissue perfusion, blood pressure, etc.) is one of the earliest stages in the progression of various chronic diseases. OBJECTIVE: The aim of this study was to determine if a difference in microvascular function existed between two metabolically healthy groups that differed in BMI and waist circumference. DESIGN: This study employed a causal comparative design, with two groups: I) normal weight (n =14, BMI 28 kg/m2). Insulin resistance in metabolic syndrome subjects is profound in spite of muscle insulin receptor and insulin-responsive glucose transporter (GLUT4) expression being nearly normal. Insulin receptor tyrosine kinase phosphorylation of insulin receptor substrate-1 (IRS-1) at Tyr896 is a necessary step in insulin stimulation of translocation of GLUT4 to the cell surface. Serine phosphorylation of IRS-1 by some kinases diminishes insulin action in mice. We evaluated the phosphorylation status of muscle IRS-1 in 33 subjects with the metabolic syndrome and seventeen lean controls. Metabolic dysfunction in skeletal muscle is a major contributor to the development of type 2 diabetes. Endurance exercise training has long been established as an effective means to directly restore skeletal muscle glucose and lipid uptake and metabolism. However, in addition to the direct effects of skeletal muscle on glucose and lipids, there is renewed interest in the ability of skeletal muscle to coordinate metabolic activity of other tissues, such as adipose tissue and liver. Niacin can reduce vascular disease risk, but its mechanism of action is controversial, and may not be dependent on systemic lipid modifying effects. This thesis tested the hypothesis that niacin directly improves endothelial cell function under lipotoxic and low oxygen conditions, as seen in ischemic conditions during metabolic syndrome, and investigated the potential mechanism involved. Human microvascular endothelial cell (HMVEC) survival was reduced by exposure to excess fatty acids under both normoxic and low oxygen conditions. Angiogenic function, as determined by tube formation on Matrigel, was impaired during fatty acid overload under either normoxic or low oxygen conditions. These effects …

Limited research regarding metabolic syndrome (MetS) has been conducted in obese African American adolescents (OAAA) using the International Diabetes Federation IDF criteria. MetS is a constellation of risk factors that increase the risk for chronic diseases and is defined as presence of any 3 or more of the 5 criteria: fasting glucose 100mg/dl, waist circumference 90th percentile, triglycerides 150 mg/dl, blood pressure (BP) 130/85 mmHg and HDL < 40mg/dl. The aim of this study was to assess the incidence of MetS in OAAA by using the IDF criteria and the relationship with diet, fast food consumption, nutrition knowledge, home food environment (HFE), and serum calcium, leptin, 1,25-dihydroxy vitamin D (1, 25 D3) levels. One hundred and fifty one OAAA (mean age: 14.3 ± 1.4 years, 65.6% girls, mean BMI: 37.4±7.2 kg/m2) in a metropolitan area took part in this study. Anthropometric data and fasting blood samples were collected. Participants completed a set of questionnaires regarding diet, fast food consumption, and nutrition knowledge. HFE was completed by the care givers. The MetS rate for OAAA was 61.6% and boys had higher MetS than girls (73.1%, 55.6% respectively). OAAA without MetS had higher serum calcium and 1,25 D3 level but no significant difference were found in food consumption, nutrition knowledge, and HFE. Total cholesterol /HDL ratio and Insulin resistance increased the odds of MetS by 55% and 10% and 1,25 D3 decrease the odds of MetS by 2%. Even though high fast food consumption, lack of nutrition knowledge and poor food environment were negatively associated with obesity, they didn't contribute to MetS. Increase serum vitamin D and calcium level might be beneficial to reduce MetS in OAAA population.

Almost all previous studies examining the associations between glycaemic load (GL) and metabolic syndrome risk have used a daily GL value. The daily value does not distinguish between peaks of GL intake over the day, which may be more closely associated with the risk of the metabolic syndrome. Protein oxidation results in loss of tryptophan, oxidation of cysteine to cysteic acid, oxidation of methionine to methionine sulfoxide and ultimately to methionine sulfone. Methionine sulfoxide has been shown to be completely unavailable to mammalian cells and promote improved insulin sensitivity, lifespan extension, weight loss induction and reduce tumor growth in rat studies. Casein was oxidized with ozone or hydrogen peroxide to reduce the cysteine and methionine levels for use in sulfur amino acid restricted diets.
