Susceptibility At High Inoculum Of Penicillin-Resistant Staphylococcus Aureus To Six Cephalosporins, Andres Suarez, Ramon Del Busto, Edward L. Quinn, Donald Pohlod Dec 1976

Susceptibility At High Inoculum Of Penicillin-Resistant Staphylococcus Aureus To Six Cephalosporins, Andres Suarez, Ramon Del Busto, Edward L. Quinn, Donald Pohlod

Henry Ford Hospital Medical Journal

Six cephalosporins (currently available for clinical use or undergoing clinical trials) were tested by tube dilution method against high inoculum (107 colony forming units) of 37 strains of penicillin resistant Staphylococcus aureus. 3.1 μg/ml of cephalothin inhibited 100% of strains while higher concentrations of cefoxitin, cephapirin, cefamandole, cephalexin and cephradine were required. The percent of strains with minimal inhibitory concentration higher than 3.1 μg/ml ranged between 10.8% (cefoxitin) and 100% (cephradine). These findings support the observation that cephalothin is less susceptible to inactivation than the other cephalosporins. Although the exact clinical implication of these findings has not been established, …

High Dose In-Continuity Irradiation Of Para-Aortic And Pelvic Nodes For Carcinoma Of The Prostate. The First Three-And-One-Half Years, Joel Elliot White, Murray Boles

Henry Ford Hospital Medical Journal

A technique for treating the para-aortic nodes to a high dose of irradiation in continuity with the pelvis has been developed at Henry Ford Hospital and recently described by one of the authors. The "nine-field technique" was initially used in late 1973 to treat two patients with advanced prostatic carcinoma who had grossly involved para-aortic nodes. Since that time, 16 additional patients with carcinoma of the prostate, one patient with carcinoma of the urinary bladder, and one patient with carcinoma of the cervix have been treated by this technique. This paper analyzes sequelae and complications of the technique and mentions …

Rationale And Results Of Anatomic Repair Of Esophageal Hiatus Hernia, Conrad R. Lam Dec 1976

Rationale And Results Of Anatomic Repair Of Esophageal Hiatus Hernia, Conrad R. Lam

Henry Ford Hospital Medical Journal

A logical method of repair of esophageal hiatus hernia based on anatomic principles was proposed nearly 25 years ago by the late Phillip Allison. There was general agreement that his operation had advantages, but because of a disappointing number of recurrences and problems with reflux, more complicated operations were devised. Some of these involved plication of the stomach around the lower esophagus (Nissen, Belsey). In a series of 562 operations for hiatus hernia at the Henry Ford Hospital, there has been no significant deviation from the Allison technic. Complicating lower esophageal rings (Schatzki) have been excised. There has been no …

Use Of Technetium-99 Myocardial Scanning For Identification Of Intraoperative Myocardial Infarctions, Virender D. Parekh, Sidney Goldstein, Donald Magillican, Daniel Marks

Henry Ford Hospital Medical Journal

Technedum 99m stannous pyrophosphate (TcPyP) myocardial scans were obtained in 42 selected nonconsecutive patients within 12 days following open heart operations for both valve replacement (19 patients) and coronary bypass (23 patients) procedures. The scans were correlated with elevation of serum lactic dehydrogenase, glutamic oxalacetic transaminase and the electrocardiograms obtained serially in the postoperative period. TcPyP scan was more specific than enzyme studies and it was more sensitive than the electrocardiogram. All new Q waves were associated with a positive scan but there were five positive scans which were not associated with new Q waves. Myotomy and direct surgical trauma …

Prolonging Oxygen Consumption During Preservation Of Canine Kidneys By The Addition Of Continuous Dialysis, Daniel Dall'olmo, Stanley G. Dienst Sep 1976

Prolonging Oxygen Consumption During Preservation Of Canine Kidneys By The Addition Of Continuous Dialysis, Daniel Dall'olmo, Stanley G. Dienst

Henry Ford Hospital Medical Journal

Kidneys were preserved by continuous pulsatile perfusion and their metabolic rate monitored by O2 consumption. Experiments were carried out to determine if dialyzing the perfusate to supply metabolic substrates would aid in kidney preservation. A control group was compared against two experimental groups at 25°C. Average preservation time before fall off of oxygen consumption differed for each group: control - 8 hours, dialysis with amino acids - 6 hours, dialysis with amino acids and cofactors - 11 hours. Results indicated that, if suitable concentrations of metabolic substrates and cofactors were used, the preserved kidneys maintained higher levels of O2 consumption …

Oral Contraceptives And Myocardial Infarction: Report Of Three Cases And Review Of The Literature, Mohsin Alam, Remigio Garcia, Ellet H. Drake Sep 1976

Oral Contraceptives And Myocardial Infarction: Report Of Three Cases And Review Of The Literature, Mohsin Alam, Remigio Garcia, Ellet H. Drake

Henry Ford Hospital Medical Journal

Three cases are presented of young women who had myocardial infarctions possibly due to coronary thrombosis related to oral contraceptive therapy. These patients had none or very few risk factors for coronary artery disease. In two of the patients, selective coronary arteriograms showed normal results on the third and tenth week after the diagnosis was established. The pathogenesis of coronary artery thrombosis in patients receiving oral contraceptives containing estrogen is still controversial, but it appears to be the result of its multifactorial effect on the platelets, coagulation factors, vessel wall, blood pressure, postprandial blood sugar and abnormalities of lipid metabolism.

The first reported on the screening results of the program, along with associated epidemiological findings. This article presents the results of referral and follow-up. During the period of March 27 through December 11, 1975, 808 people were screened; 196 (24%) were found to have uncontrolled high blood pressure and were referred to physicians for diagnosis and possible treatment for hypertension. The program is currently following up with these people and their physicians to insure successful referral and maintenance of …

Use Of The Weibull Distribution For Analysis Of A Clinical Therapeutic Study In Rheumatoid Arthritis, W. R. Mccrum, J. T. Sharp, G. B. Bluhm Sep 1976

Use Of The Weibull Distribution For Analysis Of A Clinical Therapeutic Study In Rheumatoid Arthritis, W. R. Mccrum, J. T. Sharp, G. B. Bluhm

Henry Ford Hospital Medical Journal

This paper introduces the use of the Weibull distribution function for the analysis of small samples of clinical data. It is compared with the use of the conventional analysis of variance in determining the results of a study comparing the use of gold salts and a placebo in the treatment of rheumatoid arthritis. Although the samples were small, 14 control patients and 13 who were treated, analysis of variance determined several significant differences between the two groups. The use of the Weibull distribution, however, not only confirmed these differences, but also determined several more differences between the two groups that …

Subvalvular Aortic Stenosis Associated With Dynamic Outflow Tract Obstruction: A Case Report, Armando Cesar Madrazo, Daniel T. Anbe, James J. Karo, Peter Torbey, Ellet H. Drake, Fareed Khaja, Sidney Goldstein Sep 1976

Subvalvular Aortic Stenosis Associated With Dynamic Outflow Tract Obstruction: A Case Report, Armando Cesar Madrazo, Daniel T. Anbe, James J. Karo, Peter Torbey, Ellet H. Drake, Fareed Khaja, Sidney Goldstein

Henry Ford Hospital Medical Journal

Hemodynamic and angiographic findings are described in a 27-year-old patient, who had undergone surgical operation for subvalvular membranous aortic stenosis. Left ventriculography revealed persistence or regrowth of the subvalvular fibrous ring, and hemodynamic data revealed associated dynamic obstruction of the left ventricular outflow tract. The authors emphasize the importance of uncovering associated dynamic obstruction by provocative maneuvers and use of Beta blockers.

Penetrating Abdominal Trauma: A Morbidity And Mortality Report Of A Growing Social Problem, F. Samhouri, C. Grodsinsky, T. A. Fox Jr.

Henry Ford Hospital Medical Journal

The management of penetrating abdominal wounds has become a necessity in most larger metropolitan hospitals. Most of these wounds are produced by low velocity gunshots, but some are stab wounds. Several recent advances have contributed to the more successful management of penetrating abdominal wounds. These include: 1) improvement in on-site care by various emergency medical services, 2) more rapid transportation of the injured to a facility equipped to care for such patients, 3) vigorous resuscitative and diagnostic measures, 4) prompt and adequate surgical intervention, 5) improvements in anesthesia techniques, and 6) improvements in blood banks. From 1969 to 1974, 843 …

The Association Of Acute Cerebellar Encephalopathy And Neuroblastoma, M. Reza Jahadi, John G. Whitcomb, Ratnaker Kini, Robert H. High Jun 1976

The Association Of Acute Cerebellar Encephalopathy And Neuroblastoma, M. Reza Jahadi, John G. Whitcomb, Ratnaker Kini, Robert H. High

Henry Ford Hospital Medical Journal

Acute cerebellar encephalopathy is a rare neurological disorder associated with neuroblastoma and is characterized by ataxia, somatic myoclonus and opsoclonus. The pathogenesis which underlies the relationship between the two conditions is obscure. One patient with neuroblastoma and acute cerebellar encephalopathy Is reported. The occurrence of neuroblastoma and acute cerebellar encephalopathy In this case may be coincidental but, on the basis of frequent recent reports of this association, a possible relationship between these two entities is entertained. It is therefore recommended that all patients with acute cerebellar encephalopathy be surveyed for unapparent neuroblastoma. The search should include chest and skeletal X-rays, …

Susceptibility Of Clinical Isolates Of Bacteria To Cefamandole, Cefoxitin And Cephalothin, R. Del Busto, A. Suarez, E. Quinn, D. Pohlod Jun 1976

Susceptibility Of Clinical Isolates Of Bacteria To Cefamandole, Cefoxitin And Cephalothin, R. Del Busto, A. Suarez, E. Quinn, D. Pohlod

Henry Ford Hospital Medical Journal

The in vitro susceptibility was determined of 274 isolates to cephalothin and two new antibiotics, cefamandole and cefoxitin. Cefamandole was comparable to cephalothin in preventing growth of cultures of the gram positive organisms except for penicillin-resistant Staphylococcus aureus which was more sensitive to cephalothin. Cefamandole was more active than cephalothin against all the gram negative bacteria including Haemophilus influenzae and in addition it was active against many strains of Enterobacter sp. Cefoxitin was less active than cephalothin against the gram positive organisms but it was more active against most of the gram negative bacteria. In addition. It was active against …

Immunological Aspects Of Neoplastic Diseases, V. Radhakrishna Jun 1976

Immunological Aspects Of Neoplastic Diseases, V. Radhakrishna

Henry Ford Hospital Medical Journal

The immunological approach to neoplastic disease is gaining favor because malignant tumors are associated with immunodeficient states. Mesenchymal tumors have been seen in renal allograft recipients. The discovery of carcinoembryonic antigen, alfafetoprotein and other protein antigens associated with malignant diseases are other factors. Spontaneous regression of malignant neoplasm has been attributed to an effective immunologic process in the host. Identification of tumor specific antigens may explain these spontaneous tumor regressions. It may not be too optimistic to expect tumor vaccine of some sort within the next few years. B.C.G. and other nonspecific immunostimulants are under investigation for treatment of malignant …

Productivity Of The Roll-Streak Method To Perform Anaerobic Bacteriology In The Routine Clinical Laboratory, Thomas R. Neblett

Henry Ford Hospital Medical Journal

Data presented are derived from one year of performing routine clinical anaerobic bacteriology by the roll-streak tube method. 826 specimens from 1508 (54.8%) contained anaerobes. 162 specimens were polymicrobic (19.6%) and contained 528 strains of a total of 1051 isolated (50.2 %). 479 blood specimens contained 525 strains of anaerobes. Propionibacterium, considered a contaminant, occurred 209/210 times in blood as the sole isolate. Bacteroides occurred alone or polymicrobically 308 times and comprised 54.7% of the blood isolates and 32.5% of the total anaerobic genera isolated. Clostridium, Peptococcus and Fusobacterium were the next most frequently occurring anaerobes. The roll-streak method is …

Cushing's Disease: Spontaneous Remission Following Severe Headache, Raymond C. Mellinger Mar 1976

Cushing's Disease: Spontaneous Remission Following Severe Headache, Raymond C. Mellinger

Henry Ford Hospital Medical Journal

A young woman was found to have persistent Cushing's syndrome one year after surgical removal of both hyperplastic adrenal glands. Improvement followed pituitary irradiation butshe developed skin pigmentation. Remission of the adrenal hyperfunction occurred only after several months of therapy with o,p'-DDD. Within 15 months, the recurrent disorder required a second course of DDD with resulting adrenal insufficiency. Eighteen months later, however, steroid supplements were no longer required and increasing Cortisol levels indicated still another imminent relapse. There was no enlargement of the sella turcica. The patient had received no treatment whatever for nearly nine months when she experienced a …

Infections Due To Trichosporon Cutaneum, An Uncommon Systemic Pathogen, Tom Madhavan, John Eisses, Edward L. Quinn

Henry Ford Hospital Medical Journal

Multiple isolations of T. Cutaneum from the blood stream of two patients enabled the authors to characterize the morphology, biochemical reactions and antibiotic susceptibility of T. Cutaneum. This organism appears to be an opportunistic pathogen and responds promptly to intravenous Amphotericin B or oral 5-Fluorocytosine.
