ABSTRACT Coronary artery disease leading to myocardial infarction (a.k.a. MI, heart attack) is one of the leading causes of death globally. Each year an estimated 605,000 Americans suffer a heart attack, which equates to one MI every 40 seconds. As such, MI represents one of the largest health burdens to society. The current standard of care is revascularization therapy achieved by fibrinolytics and Percutaneous Coronary Intervention (PCI); both of which re-open occluded proximal arteries to restore blood flow to the affected areas. Fundamental to treatment planning for patients that have suffered myocardial infarction are predictive biomarkers and risk factors. Important among these in terms of a patient’s treatment plan or prognosis are the contractility of the damaged myofibers, final infarct volume, and poor infarct healing rate. Proposed and developed in this thesis are techniques to predict these biomarkers and risk factors using cardiac biomechanical modelling. One of the developed techniques was a CT compatible shape optimization technique which can predict the contraction force of healthy, and stunned myofibers within 6.3% and the distribution of potentially necrotic myofibers within 10% accuracy. In spite of the controversies regarding low-dose aspirin safety and efficacy, aspirin is widely used by individuals for the primary prevention of myocardial infarction and ischemic stroke. This study assess the appropriateness of low dose aspirin use in a sample of Lebanese population. A cross sectional study was conducted using a community based questionnaire to assess each patient’s risk factors for Myocardial Infarction and ischemic stroke and possible contraindications for aspirin use. Cardiovascular disease is the leading cause of death worldwide. A myocardial infarction (MI), commonly known as a heart attack, is a major event in cardiovascular disease characterized by reduced blood flow to the heart. The ischemia and reperfusion (I/R) injury associated with an MI results in a region of dead tissue in the heart called an infarct, the size of which influences patient prognosis. In the 1980s, it was discovered that short, non-lethal episodes of I/R, termed ischemic preconditioning (IPC), can protect the heart against a subsequent MI. Global augmentation of protein O-GlcNAcylation occurs in response to a myriad of stressors and confers a survival advantage at the cellular level. This protective phenomenon has been demonstrated to mediate cardioprotection through various in vitro and in vivo studies during ischemia-reperfusion, myocardial infarction, and oxidative stress; however, relatively little is known of the regulation of protein O-GlcNAcylation. Protein O-GlcNAcylation is regulated by two antagonistic enzymes, namely, O-GlcNAc transferase (OGT) and O-GlcNAcase (OGA). Ablation of cardiomyocyte OGT, the enzyme that catalyzes the addition of O-GlcNAc to proteins, exacerbates cardiac dysfunction during infarct-induced heart failure (HF). The epicardium is a single layer of epithelial (mesothelial) cells that covers the entire heart surface, but whose function in adult mammals is poorly understood. Defining the role of epicardial cells during homeostasis, growth and injury has potential to provide new treatment strategies for human diseases that result in heart failure, due to extensive loss of viable cardiac tissue. Several studies have shown cellular replacement therapy as a treatment strategy of myocardial infarction but results have been limited. Therefore, enhancing the therapeutic potential of stem cells injected into ischemic microenvironments by novel preconditioning (PC) techniques is critical for improving cellular therapy. Recent studies have shown that inhibition of phosphodiesterase-5 (PDE-5) is a powerful strategy to precondition the heart and cardiomyocytes against ischemia/reperfusion injury. BACKGROUND: Limited information is available on trends in the incidence of and mortality due to cardiogenic shock complicating acute myocardial infarction. We studied the incidence of cardiogenic shock complicating acute myocardial infarction and in-hospital death rates among patients with this condition in a single community from 1975 through 1997. METHODS: We conducted an observational study of 9076 residents of metropolitan Worcester, Massachusetts, who were hospitalized with confirmed acute myocardial infarction in all local hospitals during 11 one-year periods between 1975 and 1997. Phosphodiesterase Type 5 (PDE5) inhibitors are cardioprotective against ischemia/reperfusion (I/R) injury. However, it remains uncertain if I/R affects PDE5. We hypothesized that generation of reactive oxygen species (ROS) during I/R leads to upregulation of PDE5, which contributes to pathological changes following acute myocardial infarction (AMI). Adult male ICR mice were subjected to 30 minutes of in vivo or ex vivo I/R. To examine the role of ROS, a subset of hearts were perfused with 100 µM hydrogen peroxide (H2O2). In this study, the kinetic, structural and functional changes of the ventriculus in dogs with myocardial infarction introduced experimentally were examined using M-Mode and two dimensional (2-D) echocardiography. 21 healthy, mixed-breed dogs of both genders (16 females and 5 males) and of various ages were used in this study. Different coronary arteries were ligated in each dog with silk sutures and the dogs were grouped accordingly: left distalis descending coronary artery (Group I, n=7), right distalis descending coronary artery (Group II, n=7) and both left and right distalis decending coronary artery (Group III, n=7).
