Permanent breast seed implantation (PBSI) is a promising breast radiotherapy technique that suffers from operator dependence. We propose and have developed an intraoperative 3D ultrasound (US) guidance system for PBSI.

A tracking arm mounted to a 3D US scanner registers a needle template to the image. Images were validated for linear and volumetric accuracy, and image quality in a volunteer. The tracking arm was calibrated, and the 3D image registered to the scanner. Tracked and imaged needle positions were compared to assess accuracy and a patient-specific phantom procedure guided with the system.

Personalized medicine has been utilized in all stages of cancer care in recent years, including the prevention, diagnosis, treatment and follow-up. Since prevention and early intervention are particularly crucial in reducing cancer mortalities, personalizing the corresponding strategies and decisions so as to provide the most appropriate or optimal medical services for different patients can greatly improve the current cancer control practices. This dissertation research performs an in-depth exploration of personalized decision modeling of cancer intervention and prevention problems. Drug resistant estrogen receptor positive (ER-positive) and triple-negative (TN) breast cancers, are prolific breast cancers with poor clinical outcome. TRA-8 is a death receptor-5 (DR5) specific agonistic antibody with tumoricidal activity in vitro and in vivo, without inducing normal hepatocyte apoptosis. ER-positive and TN breast cancers have a range of sensitivity to TRA-8 mediated cytotoxicity. TRA-8 activation of DR5 leads to DR5 oligomerization that is critical for formation of the death inducing signaling complex (DISC) at the death domain of DR5 and initiation of apoptotic signaling. Methods built on Raman spectroscopy have shown major potential in describing and discriminating between malignant and benign specimens. Accurate, real-time medical diagnosis benefits in substantial improvements through this vibrational optical method. Not only is acquisition of data possible in milliseconds and analysis in minutes, Raman allows concurrent detection and monitoring of all biological components. Besides validating a significant Raman signature distinction between non-tumorigenic (MCF-10A) and tumorigenic (MCF-7) breast epithelial cells, this study reveals a label-free method to assess overexpression of epidermal growth factor receptors (EGFR) in tumor cells.
