The European Union Aviation Safety Agency (EASA) continues to promulgate Safety Management Systems (SMS) across aviation organizations when it recently issued SMS requirements for EASA certificated Part 145 maintenance, repair and overhaul (MRO) organizations and Part 21 Design & Manufacturing (D&M) organizations. Currently, the Federal Aviation Administration (FAA) has no such rule in place for these U.S.-based organizations, which may challenge those doing business in countries where EASA policies apply. Given that a majority of the United States’ D&M and MRO population is made up of smaller organizations, it is likely a U.S. SMS regulation will require justification of the …

The purpose of the research was to create and validate a safety performance decision-making tool to transform a reactive safety model into a predictive, decision-making tool, specific to large, collegiate Title 14 of the Code of Federal Regulations (CFR) Part 141 flight training organizations, to increase safety and aid in operational decision-making. This study uses a complex network model to analyze the causes of accidents in high-speed railway operations. By identifying the key factors that led to high-speed railway accidents, hidden safety hazards were discovered. This will help improve the operational safety of the U.S. high-speed rail line under construction.

The analysis uses the regional high-speed railway network in Guangzhou, China as a case study, including the railway (including high-speed railway) accidents that occurred in the company's jurisdiction from 2013 to 2017. Passengers rank safety as a key factor in airline choice. Thus, safety performance impacts an airline's ability to attract customers. The purpose of this correlational study was to examine the relationship and difference between airline category low-cost carriers (LCCs) and full-service carriers (FSCs), geographical region, and safety performance measured by accident rates. The target population comprised all airlines in all countries that had an accident during the 14-year period 2004 to 2017. Data consisted of archival data of all global airline accidents and airline departure frequencies for the 14-year period. Currently, U.S. air carriers do not provide equipment or training necessary to mitigate the risk posed by surface-to-air fire (SAFIRE) threats. These threats consist of self-guided weapons (infrared shoulder-fired surface-to-air missiles), manually-aimed threats (small arms, recoilless grenade launchers, rockets, and light anti-aircraft artillery), and hand-held lasers. Technological solutions to counter infrared shoulder-fired missiles have been explored, but were rejected due to prohibitive equipment and maintenance costs. A lower cost option, providing air-carrier pilots with SAFIRE risk-reduction training, has not been formally addressed by the air-carrier industry or the U.S. federal government. The risk of fatigue on pilot performance and safety is well recognized in aviation. In response to increased safety concerns, the Federal Aviation Administration addressed fatigue risk for passenger airline pilots in Title 14 Code of Federal Regulations Part 117, which included accommodations to address fatigue related risk-factors such as circadian rhythm and the phenomenon commonly described as jet-lag. The rule became effective in January 2014. A survey was conducted in March and April of 2014 to better understand pilot perceptions of the new Part 117 rule.
