Permanent breast seed implantation (PBSI) is a promising breast radiotherapy technique that suffers from operator dependence. We propose and have developed an intraoperative 3D ultrasound (US) guidance system for PBSI.
A tracking arm mounted to a 3D US scanner registers a needle template to the image. Images were validated for linear and volumetric accuracy, and image quality in a volunteer. The tracking arm was calibrated, and the 3D image registered to the scanner. Tracked and imaged needle positions were compared to assess accuracy and a patient-specific phantom procedure guided with the system.

Current ultrasound contrast agents utilize microbubbles as a blood pooling agent, but the size inhibits access to small capillaries. The development of nanoscale ultrasound contrast agents can enter small capillaries of tissues and aid in the detection of diseased states. However, current nano-formulations are flushed from the body over a short period of time. We developed a nanoscale ultrasound contrast agent with increased circulation time to allow for better detection of diseased states in the microvasculature of the body. Ultrasound Elastography (UE) is an emerging set of imaging modalities used to assess the biomechanical properties of soft tissues. UE has been applied to numerous clinical applications. Particularly, results from clinical trials of UE in breast lesion differentiation and staging liver fibrosis indicated that there was a lack of confidence in UE measurements or image interpretation. Confidence on UE measurements interpretation is critically important for improving the clinical utility of UE.
